Abstract

El desarrollo de la psicosis es multifactorial. Sin embargo, la prevalencia de antecedentes de trauma en la biografía de los pacientes psicóticos es alta. Hay una considerable evidencia que sustenta la conexión entre las experiencias tempranas de malos tratos con problemas posteriores. En base a las evidencias bibliográficas hipotetizamos que, si bien, el trauma podría no ser un factor determinante en sí mismo, contribuiría en mayor o menor medida, junto con otros factores genéticos y ambientales, a la aparición de síntomas psicóticos. Se revisó sistemáticamente la literatura para identificar aquellos artículos publicados en revistas científicas sobre los antecedentes traumáticos acontecidos hasta los 16 años y su influencia en el desarrollo posterior de un primer episodio de psicosis. De los 15 artículos incluidos, diez arrojaron una relación positiva entre el haber sufrido experiencias traumáticas en la infancia y el desarrollo posterior de síntomas psicóticos, pero también de otros síntomas psiquiátricos. Esta relación se mantiene en poblaciones con abuso de sustancias. El consumo de cannabis parece influir de forma notable en el desarrollo de sintomatología psicótica independientemente de haber o no sufrido trauma. En general, el abuso sexual parece ser el trauma infantil más frecuente en los individuos que experimentan síntomas psicóticos.

Psychosis has a multifactorial development. However, the prevalence of a traumatic event in the psychotic patient´s biography is a considerable factor. There is evidence supporting the connection between early abuse experiences with future psychiatric problems. Based on the literature evidence, we hypothesized that trauma could not be a determining factor but could contribute together with other genetic and environmental factors, in the development of psychotic symptoms. We systematically reviewed the literature to identify those published articles about traumatic events occurred up to 16 years and their influence on the subsequent development of a first episode of psychosis. Ten of the 15 included articles showed a positive relationship between suffering traumatic experiences in childhood and later development of psychosis, and also other psychiatric symptoms. This relationship is also preserved in populations with substance abuse. Cannabis use appears to have a significant influence in the development of psychosis, independently of traumatic events. Overall, sexual abuse appears to be the most common trauma in individuals experiencing psychotic symptoms.
