O papel do evitamento experiencial no ajustamento emocional no tratamento de fertilização in-vitro: Estudo dos efeitos diretos, indiretos e cruzados

Abstract

Introdução: A infertilidade consiste numa experiência que tem grande impacto na vida dos casais, afetando o seu ajustamento emocional. Para lidar com esta experiência os casais evitam frequentemente as experiências perturbadoras associadas à infertilidade. O presente estudo teve como objetivo a análise do papel do evitamento experiencial no ajustamento emocional dos casais inférteis ao longo do tratamento de fertilização in-vitro. Método: A amostra inclui 57 casais inférteis durante o tratamento no Serviço de Medicina da Reprodução do Centro Hospitalar e Universitário de Coimbra. Todos os participantes preencheram escalas de autorresposta que avaliavam o impacto da infertilidade, o recurso a evitamento experiencial e o ajustamento emocional. Resultados: O evitamento experiencial mostrou ter um efeito mediador entre o impacto da infertilidade e o ajustamento emocional, tanto nas mulheres como nos seus companheiros. Verificou-se, também, um efeito significativo do recurso a evitamento experiencial por parte do marido no aumento dos níveis de sintomatologia ansiosa e sintomatologia depressiva da sua companheira. Os resultados sugerem, ainda, que as mulheres que recorrem a níveis elevados de evitamento experiencial, no início do tratamento, experienciam significativamente mais sintomatologia ansiosa, ao longo do tratamento, que aquelas que evitam em níveis baixos as experiências perturbadoras associadas à infertilidade. Conclusões: Os resultados enfatizam o papel negativo do evitamento experiencial no ajustamento dos casais em tratamento de fertilidade. Ademais, os resultados realçam a importância de considerar o impacto que o recurso a estratégias de evitamento experiencial pelo homem tem no ajustamento da companheira. Os dados salientam, assim, a relevância de considerar o casal como unidade de análise, dada a natureza diádica da infertilidade, e do estudo dos processos de regulação emocional, em detrimento do foco de análise apenas na sintomatologia decorrente da infertilidade e do seu tratamento. Torna-se pertinente o desenvolvimento de intervenções psicológicas focadas na redução do evitamento experiencial.

Introduction: Infertility is an experience that has a great impact on the lives of couples, affecting their emotional adjustment. To deal with this experience couples often ten to avoid the disturbing experiences associated with infertility. The present study aimed to analyze the role of experiential avoidance in the emotional adjustment of infertile couples throughout the treatment of in-vitro fertilization (IVF). Method: The sample included 57 infertile couples undergoing IVF treatment at the Serviço de Medicina da Reprodução do Centro Hospitalar e Universitário de Coimbra. All participants completed self-response measures that assessed the impact of infertility, experiential avoidance, and emotional adjustment. Results: Experiential avoidance was shown to have a mediating effect between the impact of infertility and emotional adjustment in both women and their partners. There was also a significant effect of the use of experiential avoidance by the husband in anxious and depressive symptoms of his partner. The results also suggest that women who report high levels of experiential avoidance at the start of treatment experience significantly more anxious symptomatology over the course of treatment than those who report lower avoidance. Conclusions: The results emphasize the negative role of experiential avoidance in the adjustment of couples in fertility treatment. In addition, the results highlight the importance of considering the impact that the use of strategies of experiential avoidance by man has in the adjustment of the partner. The data thus highlight the relevance of considering the couple as a unit of analysis, given the dyadic nature of infertility, and the study of the processes of emotional regulation, to the detriment of the focus of analysis only on the symptomatology resulting from infertility and its treatment. It is pertinent to develop psychological interventions focused on reducing experiential avoidance.
