Abstract

Neste trabalho procedeu-se a uma revisão sistemática dos estudos e investigações realizados até então, com base de dados Medline/Pubmed e Lilacs, de modo a clarificar esta problemática do espectro no que concerne a estas entidades nosológicas. Apesar de similarmente prevalentes e crónicas, denotou-se que apresentam diferentes idades de início, tal como uma distribuição por géneros divergente. Evidenciou-se uma sobreposição bastante significativa em termos etiológicos. No que concerne à fenomenologia destas patologias surgiu novamente bastante controvérsia, mas também a neuroimagem, o estudo da função neurocognitiva e o tratamento indicaram resultados conflituantes. Outro fator que sustenta a proximidade clínica destas duas perturbações é a existência de várias comorbilidades em comum e o fato de serem mais frequentes em indivíduos que já sofrem de uma das duas condições. Analogamente a nível familiar e genético se demonstrou uma concordância considerável, tal como ambas as perturbações evidenciam a possibilidade do envolvimento de respostas neuroimunes na sua génese, o que também pressupõe alguma familiaridade nosológica. Deste modo, a partilha do mesmo espectro não é passível de ser excluída, mas sustenta-se a ideia de que a serem englobados no mesmo grupo, pertençam a pólos opostos deste mesmo espectro. Ainda assim, mais estudos seriam necessários para colmatar as incongruências que permanecem por esclarecer.

In this work it has been undertaken a systematic review of studies and investigations carried out so far, with the Medline / Pubmed and Lilacs databases, in order to clarify this issue of spectrum with regard to these nosological entities (trichotillomania and obsessive-compulsive disorder). Although both are chronic and prevalent, there is evidence of different ages of onset and different distribution by gender, but a significant overlap in etiological terms. Concerning to the phenomenology, they appeared again quite controversial, but neuroimaging, study of neurocognitive function and treatment showed conflicting results. Another factor that supports the clinical proximity of these two disorders is the presence of multiple comorbidities in common and the fact that they are more frequent in individuals who already suffer from one of the two conditions. Similarly it has been shown considerable agreement at familial and genetic level and both disorders involve the possibility of its genesis in neuroimmune responses, which also assumes some nosological familiarity. Thus, sharing the same spectrum is not likely to be excluded, but it is sustained that to be included in the same group they would belong to opposite poles of the same spectrum. Still, further studies are needed to address the inconsistencies that remain unclear.
