Abstract

INTRODUÇÃO: Actualmente o sistema de saúde e os profissionais de saúde são confrontados com situações de final da vida e com os dilemas éticos subjacentes à questão da morte encarando-a como um fracasso uma vez que estão formados para curar. As questões éticas sobre a morte, o moribundo ou o direito de autonomia do doente no final da vida são das áreas mais complexas e sensíveis que surgem nesta época. OBJECTIVOS: Revisão do conceito de morte, factores determinantes e sua previsibilidade. DESENVOLVIMENTO: Os médicos fazem uma predição subjectiva da morte, não sendo possível prever o tempo restante da vida, desta forma surgem os conflitos terapêuticos na fase final da vida. Hoje tenta-se encontrar utensílios que permitam antever quando os doentes vão morrer e acompanhá-los da melhor forma neste percurso. As decisões terapêuticas são comuns na prática clínica, no entanto, a retirada ou a retenção de tratamento bem como as decisões de não tratar ou parar de tratar levanta questões éticas. Várias decisões médicas são possíveis nesta fase final e têm vindo a receber maior atenção, sendo a principal prioridade evitar a futilidade do tratamento para se focar nos cuidados paliativos. A evidência clínica nestas situações coloca ainda questões que abordam a temática da ordem de não ressuscitação e testamento vital. CONCLUSÃO: A morte é um processo com grandes exigências no seu diagnóstico e abordagem e que integra o ciclo da vida. Os profissionais e o sistema de saúde devem dignificá-lo para possibilitar uma morte tranquila ao doente, à família e à sociedade.

INTRODUCTION: Health care providers and health system are faced with end-of-life decisions and the underlying death issue. Once they are educated to heal death is considered as a failure. The ethical issues about death, the dying patient and his right to autonomy in the end of life are the most complex and sensitive subjects arising in our time. OBJECTIVES: Review the dying process, conditioning factors and predictability. DEVELOPMENT: Doctors make a subjective prediction of the patient’s death, not being able to preview the remaining lifetime. Therapeutic decisions are problematic. We try to find tools with the ability to foresee the patients’ death and provide the best possible way. The decisions are common in clinical practice, however, the withdrawal or withholding of treatment, as well as decisions about stopping the treatment, raise ethical issues about end-of-life health care. Several medical decisions are possible in this final stage and are being increasingly focused to avoid futile care in order to provide palliative care. The clinical evidence in this situation rises further questions about the Do-Not-Resuscitate orders and advanced care planning. CONCLUSION: Death is a very demanding process with a difficult approach and integrating the life cycle. The heath care providers and the health system should allow a peaceful death to the patient, the family and the society.
