A Perturbação Dismórfica Corporal (PDC) é uma condição psiquiátrica severa que tem sido identificada pela literatura como um importante fator de risco para o desenvolvimento de dificuldades alimentares (atitudes/comportamentos alimentares perturbados). No entanto, pouco se sabe acerca dos mecanismos que podem explicar esta associação. Assim, o presente estudo teve como principal objetivo explorar a relação entre sintomatologia dismórfica corporal e comportamento alimentar perturbado e verificar se essa relação é mediada por níveis mais elevados de vergonha e autocriticismo. Este estudo transversal incluiu 291 mulheres da população geral, com idades compreendidas entre 18 e 62 anos, as quais preencheram medidas de autorrelato. A análise path revelou que a sintomatologia de PDC não só tem um efeito direto sobre o comportamento alimentar perturbado, mas também um efeito indireto, mediado pela vergonha e pelo autocriticismo. O modelo path revelou um bom ajuste, explicando 38% e 31% das variâncias da vergonha interna e externa, respetivamente, 69% da variância do autocriticismo e 58% da variância do comportamento alimentar perturbado. Estes resultados contribuem para a clarificação da relação entre sintomatologia dismórfica corporal e comportamento alimentar perturbado e parecem sugerir que, em mulheres com sintomatologia de PDC, o comportamento alimentar perturbado pode surgir como uma estratégia compensatória para lidar com sentimentos de inferioridade/defetividade, particularmente na presença de experiências de vergonha e atitudes/comportamentos autocríticos. Além disso, este estudo enfatiza a importância de investir em abordagens inovadoras de tratamento/prevenção da PDC que trabalhem especificamente a vergonha e o autocriticismo, como por exemplo as Terapias Focadas na Compaixão.
Body dysmorphic disorder (BDD) is a serious psychiatric condition that has long been identified as an important risk factor for the development of eating-related difficulties. However, little is known about the mechanisms that might explain this association. Therefore, the current study aimed to explore the link between body dysmorphic symptomatology and disordered eating, and test whether this relationship is mediated by higher levels of shame and self-criticism. This cross-sectional study included 291 women from the community, aged between 18 and 62 years old, who completed self-report measures. Path analysis revealed that BDD symptomatology has not only a direct effect on disordered eating, but also an indirect effect, mediated by shame and self-criticism. The path model revealed a very good fit, accounting for 38% and 31% of internal and external shames’ variances, respectively, for 69% of self-criticism variance, and 58% of the variance of disordered eating. These findings contribute to a clarification of the relationship between BDD-related symptoms and disordered eating, and seem to suggest that in women with BDD symptomatology, disordered eating may emerge as a compensatory strategy to cope with general feelings of inferiority/defectiveness, particularly in the presence of shame experiences and self-critical attitudes/behaviours. Moreover, this study emphasizes the importance to invest in innovative treatment and prevention approaches for BDD that specifically target shame and self-criticism, such as compassion-based therapies.
