This is the Ethics Assessment Form developed by the Ethics Commission of the Department of Informatics at University of Hamburg, Germany. The commission uses this form for its own application and evaluation procedure. More specifically, the form aims to achieve two main goals: First, the form provides researchers with some guiding questions to reflect on the potential ethical implications of their research project. And second, it provides the ethics commission with the minimum relevant information about the research project required for making an ethics assessment. The commission shares the ethics assessment form here to provide a resource for other universities and departments that are also looking to implement an ethics review procedure or identify avenues for conducting more ethically responsible research.
