Perfil de mortalidad asociado a COVID 19 en pacientes ingresados en el hospital General Docente Ambato, Ecuador

Abstract

Introducción: La enfermedad por coronavirus 2019 (COVID-19) causa una enfermedad multisistémica con afectación predominantemente respiratoria como neumonía e insuficiencia pulmonar. El comportamiento letal de la enfermedad está relacionado a la presencia de comorbilidades que empeoran su pronóstico. En este estudio evaluaron varios de los factores que aumentan el riesgo de mortalidad entre los pacientes hospitalizados COVID-19 y el análisis cuantitativo de casos con mortalidad en lo que respecta a nivel local del cantón Ambato en la provincia de Tungurahua. Metodología: Se realizó un estudio con diseño retrospectivo y descriptivo en 1080 pacientes con COVID-19 de la base de datos del Hospital General Docente Ambato “HGDA” durante el periodo 25 de marzo hasta el 22 de agosto del 2020, en estos datos se han estimado las variables respuesta para identificar el impacto de la epidemia, las tasas de incidencia, mortalidad y letalidad. Resultados: De 1080 pacientes evaluados, los principales grupos afectados en nuestra población fueron el sexo masculino (56,6%; n=611), sujetos >50 años (60,9%; n=657), la presencia de comorbilidad (90,9%; n=982) y mortalidad de 34,5% (n=372). La tasa de mortalidad fue más alta en aquellos con comorbilidades (sin comorbilidad: 8,1% vs con comorbilidad: 33,5%). Conclusión: En la población analizada la tasa de mortalidad es directamente proporcional a las comorbilidades asociadas, viéndose incrementada 25% en comparación de los pacientes quienes no presentaron comorbilidades, específicamente la enfermedad cardiovascular, HTA, DM y ERC; duplican el riesgo de morir por SARS-CoV2 en comparación con los pacientes sin estas comorbilidades.
