Abstract

Se presenta una paciente de 38 años, embarazada de 32 semanas, la cual fue sometida a una cesárea electiva con ligadura tubaria bilateral y reconstrucción de pared abdominal con malla. Antecedentes personales: obesidad súper mórbida (BMI 70.6), hipertensión arterial, diabetes tipo 1, sarcoidosis hepatopulmonar, y apnea obstructiva del sueño. Antecedentes quirúrgicos 3 cesáreas. El examen físico de la columna lumbar no arrojo datos debido al panículo adiposo. El examen de la vía aérea, reveló dentición normal, mallampati III, distancia tiromentoniana 4 cm, apertura bucal 4 cm y mamas grandes. Considerando las características de la paciente se decidió como táctica anestésica, abordar la cirugía con una técnica regional peridural, la cual se realizó con éxito, a nivel l3 l4 con aguja tuohy 16g larga y jeringa de baja resistencia. Se administró en total 210 mg de lidocaína 1% con epinefrina, fentanilo 50 mcg y clonidina 25 mcg peridural. La cirugía tuvo una duración de 5 horas durante las cuales la paciente se mantuvo hemodinámicamente estable, requiriendo 0.5 mcg/kg/min de fenilefrina y c-pap para mantener sin colapso la vía aérea. Se infundieron en total 2 litros de ringer lactato. El bebe nació sin complicaciones y tanto el como la madre fueron externados a la semana del procedimiento.
