Association of Childhood Physical and Sexual Abuse with Intimate Partner Violence, Poor General Health and Depressive Symptoms among Pregnant Women

Abstract

Nous avons examiné les associations entre les abus physiques et sexuels pendant l'enfance et le risque de violence entre partenaires intimes (VPI). Nous avons également évalué la mesure dans laquelle la maltraitance infantile était associée à l'état de santé général autodéclaré et aux symptômes de la dépression antepartum dans une cohorte de femmes péruviennes enceintes. Des entretiens en personne ont été menés pour recueillir des informations sur les antécédents de maltraitance infantile et de VPI auprès de 1 521 femmes en début de grossesse. La symptomatologie dépressive antepartum a été évaluée à l'aide du Patient Health Questionnaire-9. Des procédures de régression logistique multivariables ont été utilisées pour estimer les rapports de cotes ajustés (RCa) et les intervalles de confiance à 95 % (IC à 95 %). Toute maltraitance infantile a été associée à une probabilité 2,2 fois plus élevée de VPI au cours de la vie (IC à 95 % : 1,72-2,83). Comparativement aux femmes qui n'ont signalé aucun abus pendant l'enfance, celles qui ont signalé à la fois des abus physiques et sexuels pendant l'enfance présentaient un risque de VPI physique et sexuel au cours de leur vie multiplié par 7,14 (IC à 95 % : 4,15-12,26). La probabilité de subir des abus physiques et sexuels de la part d'un partenaire intime au cours de la dernière année était 3,33fois plus élevée chez les femmes ayant des antécédents d'abus physiques et sexuels dans l'enfance que chez les femmes qui n'ont pas été abusées dans leur enfance (IC à 95 % 1,60-6,89). La maltraitance infantile était associée à une probabilité plus élevée de mauvais état de santé autodéclaré au début de la grossesse (RCa = 1,32, IC à 95 % : 1,04-1,68) et à des symptômes de dépression antepartum (RCa = 2,07, IC à 95 % : 1,58-2,71). Ces données indiquent que la violence sexuelle et physique pendant l'enfance est associée à la VPI, à une mauvaise santé générale et à des symptômes dépressifs au début de la grossesse. La forte prévalence des traumatismes infantiles et leurs effets durables sur la santé des femmes justifient des efforts de santé mondiaux concertés dans la prévention de la violence.

Examinamos las asociaciones del abuso físico y sexual infantil con el riesgo de violencia de pareja (VPI). También evaluamos la medida en que el abuso infantil se asoció con el estado de salud general autoinformado y los síntomas de depresión prenatal en una cohorte de mujeres peruanas embarazadas. Se realizaron entrevistas en persona para recopilar información sobre el historial de abuso infantil y la VPI de 1.521 mujeres durante el embarazo temprano. La sintomatología depresiva anteparto se evaluó mediante el Cuestionario de salud del paciente-9. Se utilizaron procedimientos de regresión logística multivariable para estimar los odds ratios ajustados (aOR) y los intervalos de confianza del 95% (IC del 95%). Cualquier abuso infantil se asoció con un aumento de 2,2 veces en las probabilidades de IPV de por vida (IC del 95%: 1,72-2,83). En comparación con las mujeres que no denunciaron ningún abuso infantil, las que denunciaron ambos, el abuso físico y sexual infantil tuvieron un riesgo de por vida de 7,14veces de violencia física y sexual (IC del 95%: 4,15-12,26). Las probabilidades de sufrir abuso físico y sexual por parte de una pareja íntima en el último año fueron 3,33veces mayores entre las mujeres con antecedentes de abuso físico y sexual infantil en comparación con las mujeres que no sufrieron abuso cuando eran niñas (IC del 95%: 1,60-6,89). El abuso infantil se asoció con mayores probabilidades de mal estado de salud autoinformado durante el embarazo temprano (aOR = 1.32, IC del 95%: 1.04-1.68) y con síntomas de depresión antes del parto (aOR = 2.07, IC del 95%: 1.58-2.71). Estos datos indican que el abuso sexual y físico infantil se asocia con IPV, mala salud general y síntomas depresivos en el embarazo temprano. La alta prevalencia del trauma infantil y sus efectos duraderos en la salud de las mujeres justifican esfuerzos concertados de salud mundial para prevenir la violencia.

We examined associations of childhood physical and sexual abuse with risk of intimate partner violence (IPV). We also evaluated the extent to which childhood abuse was associated with self-reported general health status and symptoms of antepartum depression in a cohort of pregnant Peruvian women.In-person interviews were conducted to collect information regarding history of childhood abuse and IPV from 1,521 women during early pregnancy. Antepartum depressive symptomatology was evaluated using the Patient Health Questionnaire-9. Multivariable logistic regression procedures were used to estimate adjusted odds ratios (aOR) and 95% confidence intervals (95%CI).Any childhood abuse was associated with 2.2-fold increased odds of lifetime IPV (95%CI: 1.72-2.83). Compared with women who reported no childhood abuse, those who reported both, childhood physical and sexual abuse had a 7.14-fold lifetime risk of physical and sexual IPV (95%CI: 4.15-12.26). The odds of experiencing physical and sexual abuse by an intimate partner in the past year was 3.33-fold higher among women with a history of childhood physical and sexual abuse as compared to women who were not abused as children (95%CI 1.60-6.89). Childhood abuse was associated with higher odds of self-reported poor health status during early pregnancy (aOR = 1.32, 95%CI: 1.04-1.68) and with symptoms of antepartum depression (aOR = 2.07, 95%CI: 1.58-2.71).These data indicate that childhood sexual and physical abuse is associated with IPV, poor general health and depressive symptoms in early pregnancy. The high prevalence of childhood trauma and its enduring effects of on women's health warrant concerted global health efforts in preventing violence.
