La valutazione dell’Alessitimia con la Toronto Structured Interview for Alexithymia (TSIA): applicazione in un campione di soggetti obesi

Abstract

Introduzione: L’alessitimia, quale deficit della regolazione emotiva, è considerato un fattore di rischio trasversale alla patologia mentale e somatica tra cui l’obesità. Lo strumento maggiormente impiegato per la valutazione dell’alessitimia è la 20-item Toronto Alexithymia Scale (TAS-20) un questionario self report. Per superare i principali limiti della TAS-20, ovvero la richiesta di auto-valutare i propri processi emotivi e l’influenza della desiderabilità sociale su quanto valutato, è stata sviluppata un’intervista strutturata: la Toronto Structured Interview for Alexithymia (TSIA). Obiettivo di questa ricerca è quello di valutare in un campione di soggetti obesi la relazione tra alessitimia, peso e sintomatologia psicofisica, rilevando eventuali differenze tra i due strumenti di valutazione dell’alessitimia impiegati. Metodo: A 50 soggetti obesi (età m=43,08 ds=11,01; peso m=118,4 Kg ds=23,17) sono stati somministrati: Symptom Checklist-90-Revised (SCL-90-R); TAS-20; TSIA e una scheda socio-anamnestica. Risultati: Dalle analisi dei dati è emersa una correlazione significativa tra TAS-20 e TSIA (r=.31; p<.05). I dati evidenziano come la TSIA risulti positivamente correlata con il peso (r=.38; p<.01) mentre per la TAS-20 non emergono significatività. Al contrario, la TAS-20 correla positivamente con la sintomatologia psicofisica (GSI) (r=.43; p<.01), mentre per la TSIA non si evidenziano significatività. Infine, confrontando i punteggi totali della TAS-20 e TSIA di questo campione con quelli del campione normativo tratto dalla popolazione generale emerge che: con la TAS-20 i soggetti obesi risultano significativamente meno alessitimici del campione normativo (t=-3.35; p<.01), mentre con la TSIA risultano significativamente più alessitimici (t=4.80; p<.01). Conclusioni: I risultati mostrano una relazione differente tra alessitimia, peso e sintomi psicofisici a seconda della tipologia strumento utilizzato (TAS-20/TSIA). Questo dato ci porta ad ipotizzare l’utilità di un assessment multimetodo dell’alessitimia per questa specifica popolazione clinica. Ulteriori studi sono necessari.
