Intolerancia a la lactosa y efectos en la microbiota

Abstract

La intolerancia a la lactosa es una enfermedad provocada por la mala absorción del disacárido lactosa y que produce diferentes síntomas en el paciente tales como diarrea, borborigmos, dolor abdominal e inflamación. Dentro de los tipos que existen, en esta revisión nos centraremos en la intolerancia primaria, causada principalmente por la disminución de la expresión de la enzima lactasa a partir del destete. Esta disminución de lactasa impide la hidrólisis de la lactosa en glucosa y galactosa, dando lugar a una mala fermentación en el colon. Estudios recientes han demostrado la existencia de una relación directa entre la composición bacteriana de la microbiota y los síntomas que se sufren debido a la intolerancia a la lactosa. Entre las causas que intervienen en la alteración de la composición de la flora bacteriana se encuentran tanto factores ambientales (nutrición, antibióticos, modo de vida, edad, estrés, etc.) como genéticos. En este trabajo se tiene muy en cuenta la nutrición ya que es un factor determinante puesto que la lactosa está presente en gran cantidad de alimentos ya sean considerados lácteos o no. Actualmente, se están llevando a cabo ensayos clínicos con la finalidad de dar con un tratamiento eficaz para la intolerancia a la lactosa. En esta revisión bibliográfica hablaremos de la adaptación colónica, la utilización de galactooligosacáridos y del uso de probióticos, prebióticos y simbióticos. Destacaremos estos últimos como los más utilizados y principal objeto de este trabajo y mostraremos cómo su función es la estimulación del crecimiento de cepas bacterianas en la flora que son capaces de mejorar la fermentación de la lactosa y, por tanto, dar lugar a una mejor digestión de ésta.
