Abstract

Food supplements (FS) were defined in Turkey in 2013 by the Ministry of Food, Agriculture, and Livestock in the Turkish Food Codex Communiqué on Supplementary Foods”. Since it is stated as not a drug in the communique, it can be obtained directly from anywhere without a doctor's prescription. Determining the rate of use of FSs, which can be toxic and have drug-drug and drug-food interactions, will shed light on a new public health problem. Aim: The aim of the study is to determine the knowledge, attitudes, and behaviors of family-physicians (FP) and family-health-personnel (FHP) working in Family Health Centers (FHC) in Kocaeli/ Gebze about food-supplements and the factors affecting their use of FS. Material and Method: It is a cross-sectional study conducted between July 2021 and January 2022. Research permissions were obtained from Marmara University Ethics Committee (June 4, 2021/ September, 2021.700), Kocaeli Provincial Health Directorate, and informed consent was obtained from the participants. The data were collected from the employees in all FHCs (N=216) in Gebze, the data were collected face to face with the information form created by the researcher and analyzed in SPSS. The statistical significance level was set at p<0.05. Results: In the study, 80 FP and 96 FHP aged 40.61±8.18 years were interviewed (72.7% women). FS use was prevalent in 51.3% of FP and 47.9% of FHP (p:0.660). FS was recommended by 66.3% of FPs and 35.4% of FHPs to their clients/ patients (p:0.001). 51.7% of the participants who used FS used it more during the Covid-19 pandemic process. 79.3% of those who recommended FS to their applicants suggested more during the pandemic. The median knowledge of FS score of the participants was 50.0 (range: 0-90). FP’s median knowledge of Fs score was 60.0 (range:0-90), FHP’s was 40.0 (range:0-80, p:0.001). It was determined that as the time spent in the profession increased, the FS knowledge scores decreased (r:-0.210, p:.005), and as the weekly exercise time increased, the FS score increased (r:0.162, p:0.031). Conclusion: According to our findings, half of the employees in Gebze FHCs used any type of FS during the pandemic, and the majority of family physicians who used FS recommended its use to their applicants. The decrease in FS knowledge as time spent in the profession increases has revealed the need for in-service FS training on a regular basis.

Türkiye’de gıda takviyeleri (GT), 2013’te Gıda, Tarım ve Hayvancılık Bakanlığı tarafından “Türk Gıda Kodeksi Takviye Edici Gıdalar Tebliği”nde tanımlanmıştır. Tebliğde “ilaç değildir” seklinde belirtildiğinden hekim reçetesi olmaksızın her yerden doğrudan temin edilebilmektedir. Toksik etkileri olabilen, ilaç-ilaç ve ilaç-besin etkileşimleri de olabilen GT’lerin, ne oranda kullanıldığının saptanması, yeni bir halk sağlığı sorununa ışık tutabilecektir. Amaç: Kocaeli/ Gebze’deki Aile Sağlığı Merkez’lerinde (ASM) görev yapan aile-hekimleri (AH) ve aile-sağlığı-elemanlarının (ASE) gıda-takviyeleriyle (GT) ilgili bilgi, tutum ve davranışlarıyla GT kullanımlarını etkileyen faktörlerin belirlenmesi amaçlanmıştır. Gereç/ Yöntem: Temmuz 2021-Ocak 2022 tarihleri arasında yürütülen kesitsel bir araştırmadır. Marmara Üniversitesi Etik Kurulu’ndan (04.06.2021/ 09.2021.700), Kocaeli İl Sağlık Müdürlüğü’nden araştırma izinleri, katılımcılardan da bilgilendirilerek onamları alınmıştır. Gebze’deki tüm ASM’lerde çalışanlardan (N=216), yüz-yüze, araştırmacı tarafından oluşturulmuş bilgi formuyla veri toplanmış, SPSS’de analiz edilmiştir. İstatistiksel anlamlılık düzeyi olarak p<0.05 kabul edilmiştir. Bulgular: Araştırmada yaşları 40,61±8,18 yıl olan 80 AH, 96 ASE’yle görüşülmüştür (%72,7’si kadın). AH’nin GT kullanım prevalansı %51,3, ASE’nın, %47,9’du (p:0.660). AH’nin %66,3’ü, ASE’nın %35,4’ü danışanlarına/ hastalarına GT öneriyordu (p:0.001). GT kullanan katılımcıların %51,7’si Covid-19 pandemi sürecinde daha fazla kullanmıştı. Başvuranlarına GT önerenlerin %79,3’ü pandemide daha fazla önermişti. Katılımcıların ortanca GT bilgi-puanları 50,0’dir (range:0-90). AH’nin 60,0 (range:0-90), ASE’nın 40,0’tı (range:0-80, p:0.001). GT puanları meslekte geçen süre arttıkça azalırken (r:-0.210, p:0.005), haftalık egzersiz süresi arttıkça arttığı saptandı (r:0.162, p:0.031). Sonuç: Araştırmamızda Gebze’deki ASM’lerde çalışanların yarısının pandemi süresince herhangi bir GT kullandığı ve kullanan AH’nin çoğunluğunun GT kullanımını başvuranlarına önerdiği saptanmıştır. Meslekte geçen süre uzadıkça GT bilgisinin azalması periyodik olarak GT ile ilgili hizmet-içi eğitim ihtiyacını ortaya koymuştur.
