Abstract

Çalışmamızda 180 adet tek köklü üst kesici çekilmiş insan dişi kullanıldı. Örnekler üç gruba ayrılarak kök kanalları; step-back, Mtwo ve Protaper teknikleri ile prepare edildikten sonra lateral kondensasyon tekniğiyle dolduruldu. Ardından bu üç ana grup; rezeksiyon ve retropreparasyon işlemleri gerçekleştirilmek üzere üç alt gruba ayrıldı. Birinci grubun rezeksiyonu ve retropreparasyonu Er:YAG lazerle yapıldı. İkinci grubun rezeksiyonu Er:YAG lazer, retropreparasyonu ultrasonik retrotiple gerçekleştirilirken; üçüncü grubun rezeksiyon ve retropreparasyonunda konvansiyonel teknik uygulandı. Ardından bu gruplar, numunelerin apikal dentin yüzeyinde oluşan mikroçatlakların stereomikroskop altında incelenmesi ve retrograd kavitelerin apikal sızıntılarının sıvı filtrasyon tekniği ile değerlendirilebilmesi için iki alt gruba daha ayrıldı. Stereomikroskop gruplarındaki örneklerden her gruptan 8er diş seçilerek SEM analizleri yapıldı. Çalışmamızın sonuçlarına göre; hem rezeksiyon hem de retropreparasyon sonrası toplam çatlak sayıları değerlendirildiğinde gruplar arası anlamlı farklılık bulunamadı (p>0.05). Apikal rezeksiyonları konvansiyonel teknikle gerçekleştirilen grupların rezeksiyon sürelerinin, lazerle gerçekleştirilenlere göre anlamlı derecede daha kısa olduğu belirlendi (p=0.01). Retropreparasyon tekniğine göre, preparasyon süreleri arasında istatistiksel olarak anlamlı farklılık bulundu (p=0.000). En kısa preparasyon süresi konvansiyonel grupta; en uzun süre ise lazer grubunda tespit edildi. Retropreparasyon tekniğine göre kanal duvarlarında güta-perka artığı en fazla ultrasonik grupta, en az ise konvansiyonel grupta gözlendi. Kök kanal preparasyonları aynı, retropreparasyonları farklı olan gruplar arasında en fazla sızıntı ultrasonik gruplarında, en az sızıntı ise lazer gruplarında tespit edildi.Anahtar kelimeler: apikal rezeksiyon, retrograd kavite, ultrasonik, Er:YAG lazer,sıvı filtrasyon.

One hundred and eighty maxillary anterior extracted human teeth were used. The samples were divided into three groups and the root canals were prepared by step-back, Mtwo or Protaper techniques and obturated with lateral condensation. These main three groups were divided into three subgroups for resection and retropreparation procedures. In the first group, resection and retropreparation were performed by Er:YAG laser, in the second group resection was performed by Er:YAG laser, retropreparation was performed by ultrasonic retrotip. In the third group, resection and retropreparation were performed by conventional technique. Then, these groups again were divided into two subgroups to examine microcrack formation on the apical surfaces by stereomicroscope and to evaluate the apical leakage of the retrograde cavities by fluid filtration method. From the samples of stereomicroscope groups eight teeth were chosen and SEM analyzes were done. After resection and retropreparation, the total number of cracks were evaluated and the difference between groups was not found significant (p>0.05). The preparation time of conventionally resected group was significantly shorter than the laser group (p=0.01). According to the retropreparation techniques, the preperation times between groups were significantly difference (p=0.000). The shortest preparation time was determined for conventional group while the longest was found for the laser group. The largest amount of the gutta-percha remnants on the cavity walls was determined in ultrasonic groups, the least was in conventional groups. Among the groups prepared by same root canal preparation technique but with different retropreparation technique; the largest amount of apical leakage was found in ultrasonic groups while the least was for laser groups.Keywords: apical resection, retrograde cavity, ultrasonic, Er:YAG laser, fluidfiltration
