Abstract

Allerjik astım gelişmiş ülkelerde popülasyonun % 25’ den fazlasını etkileyen kronik bir havayolu hastalığıdır. İlaç tedavisi ve allerjenle temastan kaçınma allerjik hastalıkların kontrolünde etkili olmaktadır. Allerjen spesifik immünoterapi ise sadece allerjik hastalıkların semptomlarını tedavi etmekle kalmaz aynı zamanda hastalığın altında yatan sebeplerini de tedavi edebilir. Sublingual immünoterapinin (SLİT) allerjik hastaların tedavisinde efektif olduğu gösterilmiştir. SLİT’in tam olarak mekanizması hala bilinmemektedir, fakat oral mukozada bulunan antijen sunan hücrelerin (ASH) T regülatör hücrelerin IL-10 üretimini arttırarak allerjik immün cevabı baskılamalarına neden olduğu bilinmektedir. Bu çalışmaya ev tozuna duyarlı olan 4-10 yaş arası 10 astımlı hasta alındı. Çalışmaya alınan hastalar iki gruba ayrıldı. İlk gruba tedavi için sublingual immünoterapi ve farmakoterapi verildi (n=5). İkinci gruba tedavi için farmakoterapi verildi (n=5). Çalışmanın başında ve 8 ay sonra her iki gruptaki hastalardan PKMH izolasyonu yapıldı. İzole edilen PBMC’ ler Derp1, Betv1, PHA ile kültüre edildi ve kültürün 5. gününde alınan süpernatanlarında IL-5, IL-10, IL-13 ve IFN-γ sitokinleri analiz edildi. Çalışmamızın diğer bölümünde her iki gruptaki hastalardan ASH ve CD4+ T hücreleri magnetik ayırım ile ayrıldı. Ayrılan hücreler Derp1 antijeni ile çapraz kültür yapıldı. Çapraz kültürün 5. gününde alınan süpernatanlarda IL-5, IL-10, IL-13 ve IFN-γ sitokinleri analiz edildi. Analizler sonucunda SLİT ve farmakoterapi tedavisi alan hastalardan izole edilen ASH ile sadece farmakoterapi alan hastalardan izole edilen CD4+ T hücrelerinin Derp 1 ile yapılan kültüründe IL-10 seviyesi yüksek bulunmuştur. Sonuç olarak; SLİT grubundan izole edilen ASH in vitro olarak allerjen ile uyarıldığında kontrol grubundan izole edilen CD4+ T hücresinden IL-10 salınımını tetiklemektedir. THE ROLE OF DENDRITIC CELL IN MUCOSAL IMMUNOTHERAPHY The most common form of allergy IgE mediated hypersensitivity affects more than 25% of the population in industrialized countries. Asthma is currently defined as a chronic inflammatory disease in which allergens are often implicated as causative and triggering factors of respiratory attacks. T cell and antigen presenting cell are functionary to asthma mechanisms. Although we have some ideas about sublingual immunotherapy, we don’t have any idea of sublingual immunotherapy effect mechanism. In this study, APC’s effects to the CD4+ T cell which the children had treatment SLİT that we evaluated the effect that SLİT on the APC and/ or CD4+ T cell. In this study ten patients (age range 4-10 years) suffering from mild to moderate asthma with single sensitization to mite allergen were enrolled. Patients divided in two groups. First group had given sublingual immunotherapy and pharmacotherapy for treatment (n=5). Second group had given pharmacotherapy for treatment ( n=5). At the beginning of and 8 months later PBMC were isolated and cultured with Derp1, Betv1, PHA and supernatants were collected at the fifth day and IL-5, IL-10, IL-13 and IFN-γ cytokines level analyzed . In a separate experiment APC and CD4+ T cell were separated with magnetic separation from both two groups and cross culture were performed with Derp1 stimulation Supernatants were collected at the fifth day IL-5, IL-10, IL-13 and IFN-γ cytokines level analyzed. We concluded that antigen delivery via sublingual route stimulates toleregenic antigen presents cell, which stimulates IL-10 secretion in untreated CD4+ T cells.
