Miyofasyal ağrı sendromunda kinesiotaping uygulamasının etkinliği

Abstract

Bu çalışmanın amacı miyofasyal ağrı sendromlu (MAS) olgularda kinesiotaping uygulamasının ağrı ve fonksiyonel seviye üzerine etkilerini araştırmaktır. Çalışmaya miyofasyal ağrı sendromlu 42 olgu katılmıştır. Olgular rastgele olarak 2 gruba ayrılmıştır. Birinci gruba (n=21, Xyaş=35,2±11,5 ) sıcak paket ve transkutanöz elektiriksel sinir stimülasyonu (TENS) uygulanmış, ikinci gruba (n=21 Xyaş=38,9±9,8 ) ise bu uygulamalara ek olarak kinesiotaping yapılmıştır. Olgular toplam 10 seans olacak şekilde 2 hafta tedaviye alınmıştır. Her iki gruba da tedavi sonrası ev egzersiz programı verilmiştir. Olgular tedavi öncesi, tedavi sonrası ve tedaviden 4 hafta sonra değerlendirmeye alınmıştır. Boyun eklem hareket açıklığı (EHA) inklinometre ile, omuz EHA ise gonyometre ile değerlendirilmiştir. Visual analog skala (VAS) ile ağrı, pressure algometre ile ağrı eşiği değerlendirilmiştir. Olguların emosyonel durumları Beck depresyon skalası ile fonksiyonel durumları ise boyun ağrı ve disabilite skoru ile değerlendirilmiştir. Kinesiotaping uygulanan olgularda VAS değerleri, omuz EHA'dan abduksiyon, adduksiyon, fleksiyon, eksternal ve internal rotasyon, boyun EHA' dan fleksiyon, ekstansiyon, sağ-sol lateral fleksiyon ve rotasyon, algometrik değerlendirmede üst trapez 1. nokta ve üst trapez 2. nokta, boyun ağrı ve disabilite skoru ve Beck depresyon skalası değerlerinde istatistiksel olarak anlamlı derecede fark bulunmuştur. Çalışma sonunda kinesiotaping uygulamasının MAS'ta boyun ve omuz EHA, ağrı, ağrı eşiği, emosyonel durum ve fonksiyonel seviye üzerinde olumlu etkileri olduğu gözlenmiştir.

The aim of this study was to invastigate the effect of kinesiotaping on pain and functional level among subjects with myofascial pain syndrome. The participants of the study were 42 patients diagnosed myofascial pain syndrome. Subjects were randomly allocated to two groups. Hot pack and TENS was applied to first group (n=21, Xage=38,9±9,8). Kinesiotaping was placed additionally to second group (n=21, Xage=35,2±11,5). Subjects were treated for 10 seances during 2 weeks. Both groups were performed home exercises programe. Measurements were made before and after treatment. Also, measurement repeated after 4 weeks of treatment. Neck ROM was determined by inclinometer, shoulder ROM was evaluated by goniometer. Pain was determined by VAS, pain threshold was measured by pressure algometer. Emotional status was evaluated by beck depresion scale, functional level was determined by neck pain and disability score. The difference between before and after treatment VAS scores of kinesiotaping group was found statictically significant. The significant difference was identified for abduction, adduction, flexion, external and internal rotation of shoulder and also for flexion, extension, rotation, right and left lateral flexion of neck in knesiotaping group. Kinesiotaping group experienced significant increase neck pain and disability score and beck depresion scale before and after treatment. It was concluded that kinesiotaping had possitive effect on neck and shoulder ROM, pain, pain treshold, emotional status and functional level among subjects with myofascial pain syndrome.
