Abstract

In this study, we investigated the effectiveness of kinesiotaping applications on the carpal tunnel syndrome (CTS) symptoms. 72 hands of 44 patients, clinical and electrodiagnostically proven CTS were randomly divided in to 3 groups. All groups treated with hot- pack and transcuteneous electrical nerve stimulation (TENS) five times per week for two weeks, kinesiotaping group also treated with kinesio applications, splint group also use wrist splint day and night. Patients were evaluated by visual analog scale (VAS), Semmes- Weinstein monofilament, dynamometer, pinchmeter, Boston questionnaire before the treatment, after the fifth and tenth sessions of treatment, 4 weeks later and 12 weeks later. Nerve conduction studies were applied to the patients before the treatment and at the end of 12 weeks. There were statistically significant differences in all groups among VAS, dynamometer and functional capacity scale. Splint group showed significant differences for parameters of VAS, functional capacity and symptom severity scale at 4 weeks later the treatment. Kinesiotaping group showed significant differences for parameter of functional capacity scale at the end of 12 weeks treatment and for parameter of symptom severity scale at the and of fifth and tenth sessions treatment. Control group showed significant differences for parameter of symptom severity scale at 12 weeks later the treatment. There were no statistically significant differences in all groups among dynamometer, pinchmeter and Semmes-Weinstein monofilament test. Kinesiotaping group showed significant differences for parameter of motor distal latancies and palm-wrist sensory nerve velocity 12 weeks later the treatment.As a result, in this study it is seen that kinesiotaping applications have diminished CTS symptoms and provided improvement in the electrodiagnostic parameters. However any significant differences are not determined over other groups. For that reason, it can be claimed that ?kinesiotaping applications? is an alternative method for the treatment of mild and moderate CTS cases. There is a need for further studies investigating different kinesiotaping applications and different physical therapy modalities combinations.

Bu çalışmada kinesiotaping uygulamasının karpal tünel sendromu (KTS) tedavisindeki etkinliği araştırıldı. Klinik ve elektronöromiyografik (ENMG) tetkik ile hafif ve orta dereceli KTS teşhisi konulan 44 hastanın 72 eli rastgele yöntemle 3 gruba ayrıldı. Tüm gruplara yüzeyel ısı ajanı olan sıcak paket ve transkutanöz elektriksel sinir stimülasyonu (TENS) uygulandı. Kinesiotaping grubuna, bu modalitelerle birlikte kanal içi basıncı ve ağrıyı azaltacak, kas gücünü ve kan dolaşımını arttıracak şekilde kinesiotaping uygulaması yapıldı. Splint grubu da gece- gündüz el-el bileği splinti kullandı. Fizik tedavi haftada 5 seans olmak üzere 2 hafta uygulandı. Hastalar tedavi öncesi, 5. seans ve 10. seans sonunda, 4. ve 12. haftalarda; vizüel analog skala (VAS), Semmes-Weinstein monofilamentleri, dinamometre, pinchmetre ve Boston anketi ile değerlendirildi. Tedavi öncesi ve 12 hafta sonunda ENMG tetkiki yapıldı. VAS, dinamometre ve fonksiyonel kapasite skalası sonuçlarında tüm gruplarda istatistiksel olarak anlamlı düzelme saptanırken, splint grubunda 4. hafta sonunda VAS, fonksiyonel kapasite ve semptom şiddet skalalarında diğer gruplara göre istatistiksel olarak anlamlı düzelme saptandı. Kinesiotaping grubunda 12 hafta sonunda fonksiyonel kapasite skalasında, 5. ve 10. seans sonunda semptom şiddet skalasında diğer gruplara göre anlamlı derecede azalma saptandı. Kontrol grubunda 12 hafta sonunda semptom şiddet skalası değerinde diğer gruplara göre anlamlı derecede azalma saptandı. Dinamometre, pichmetre ve Semmes-Weinstein monofilament testinde gruplar arası fark saptanmadı. ENMG parametrelerinden motor distal latans süresi ve avuç-bilek duyu iletim hızında kinesiotaping grubunda başlangıç ve 12. Hafta arasında anlamlı düzelme saptanırken gruplar arasında fark saptanmadı.Sonuç olarak bu çalışmada kinesiotape uygulamasının KTS semptomlarını azalttığı, elektrodiagnostik parametrelerde iyileşmeler sağladığı görüldü. Bu nedenle kinesiotape uygulamasının hafif ve orta dereceli KTS tedavisinde uygun bir tedavi alternatifi olabileceği söylenebilir. Bu konuda gelecekte yapılacak çalışmalarda; değişik kinesiotape uygulamaları ve farklı tedavi modalitelerinin kombine edilerek araştırılması yararlı olabilir.
