Abstract

The cesarean section ratio in our country is 40.8% and especially in privatehospitals, this ratio is at the very high (50%).The data of the research had applied at the International Hospital and EuropeFlorence Nightingale Hospital after taking the necessary permissions from theadministrators of the hospitals which are taken into research scope.Form I applied inorder for learning sociodemographic and obstetric data, by analysing the files ofprimipara mothers (n=498) who had given birth via cesarean section, primarilybetween the years 2001-2006. Furthermore, at the dates January-June 2007,primipara mothers (n=200) who accepted the research and gave birth in bothhospitals, their files were analysed (Form I) and post op (Form II) the first day hadapplied in order to learn their opinions. Evaluating data was done in the environmentof computer.In our study, the ratio of cesarean section is too high (67%). The reasons do notbecome indication, have been becoming prominent when is decided on to cesareansection. Fear of birth of mother candidate ( 29%) , the age of being mother for thefirst time going up (58.5%), increasing the level of socioeconomic, had caused thewomen to prefer cesarean section, and most of the doctors (59%) had accepted thisdesire affirmatively.Our cases, in a big ratio (archieve 77.5%, survey 68%) had decided to cesareansection birth in prenatal (antenatal) control. In our research in the ratios (archieve35%, survey 57.4%), indications of giving birth via cesarean section had notasserted.At the end of the study, the causes such as whether getting increasing thematernal morbitidy or its cost getting high, in order to decrease the ratio, is necessaryto bring into force of health policies at the level of nation. Furthermore, studies3should be done to investigate the causes of differences between, by comparing theratios at state , university and private hospitals.Keywords: Cesarean section, The ratios of cesarean section, Indications

Bu çalısma Özel Hastanelerde Sezaryenle Dogum Yapılma Durumunuincelemek amacıyla retrospektif ve tanımlayıcı olarak planlanmıstır. Ülkemizdesezaryen oranı %40.8 olup, özellikle özel hastanelerde bu oran çok yüksektir. (%50)Arastırmanın verileri, arastırma kapsamına alınan hastanelerin yöneticilerindengerekli izinler alındıktan sonra (International Hospıtal ve Avrupa FlorenceNıghtıngale Hastanesi) toplanmıstır. Öncelikle 2001-2006 yılları arasında sezaryendogum yapmıs primipar annelerin (n=498) dosyaları incelenerek, sosyodemografikve obstetrik bilgileri ögrenmek amacıyla ( Form I) uygulanmıstır.Ayrıca 2007 ocak-haziran tarihlerinde her iki hastanede yeni sezaryen dogumyapan ve arastırmayı kabul eden (n=200) primipar annelerin önce dosyalarıincelenmis (Form I ) ve düsüncelerini ögrenmek amacıyla da post op 1.gün ( Form II)uygulanmıstır. Verilerin degerlendirilmesi bilgisayar ortamında yapılmıstır.Çalısmamızda; sezaryen oranı oldukça yüksektir. (%67) Sezaryene kararverilmesinde endikasyon olmayan nedenler öne çıkmaktadır. Anne adayının dogumkorkusu (%29.5), ilk kez anne olma yasının yukarılara kayması (%58.5),sosyoekonomik seviyenin yükselmesi kadınların sezaryeni tercih etmesine nedenolmus, hekimlerin çogu da (%59) bu istegi olumlu karsılamıstır. Olgularımız büyükoranda (arsiv grubu %77.5, çalısma grubu%68), dogum öncesi kontrollerde sezaryendoguma karar vermislerdir. Arastırmamızda (arsiv grubu%35,çalısma grubu %57.4)oranlarında sezaryen dogum endikasyonları belirtilmemistir.Çalısma sonucunda; gerek maternal morbiditeyi arttırması , gerekse maliyetininyüksek olusu gibi nedenlerle oranının düsürülebilmek amacıyla ulusal düzeyde etkinsaglık politikalarının yürürlüge konması gereklidir. Ayrıca Devlet, Üniversite ve özelhastanelerdeki oranları karsılastırmak aradaki farkların sebeplerini arastırmakamacıyla çalısmalar yapılmalıdır.Anahtar kelimeler: Sezaryen, Sezaryen oranları, Endikasyonları
