Abstract

This research is a follow up study. That aims to define the level of adherence of the patients who applied to cardiology polyclinic of Kırklareli State Hospital and diagnosed as hypertension for the first time. This study also defines the factors affecting adherence.Two hundred and ten patients who applied to cardiology polyclinic in March, April and May 2007 and who had the necessary criterion took part in this study. No sample was chosen.It aimed to reach all the patients. Three months after the diagnosis, patients were asked to come to the control. Twelve patients were not reached so their follow up study was not completed. The ratio of the completed ones was 94%.Diagnosis of hypertension were used for the patients who had high tension at least for a week and whose tension was higher than the normal value after having a rest in the polyclinic. Phasing of hypertension was done according to the JNC7 report. Demographic features and behavioral variables were collected with the questionnaires applied to the patients when they first came to the polyclinic and had the diognosis for the first time. To evaluate the adherence, third questionnaire was applied to the patients three months later. Adherence was measured in three ways 1)Adherence to medicine 2) Adherence to treatment without medicine 3) Control adherence. To define the relationship between the variables in analyses Ki-Square, SamplesT and McNemar tests were used.In this study adherence rate to medicine was found as %62. Among the reasons of not using the medicine ?forgetting to take the medicine? came first (73%). Control adherence was higher than adherence to medicine (82%). A meaningful relationship was determined between adherence to medicine and control medicine (p<0,001). Among the suggestions without medicine treatment ?decreasing the salt intake? was carried out most (83%). At the end of three months, all the participants? body mass index decreased( p<0,001). The decrease was found to be more meaningful for the patients who had both adherence to medicine and control adherence (p<0,05). Blood level control rate was found as 82% and a meaningful relationship was determined between blood pressure control and medicine control adherence (p<0,001)Adherence to hypertension treatment is directly proportional to the success of treatment. It should be told the patients that success can not be reached only with the medical treatment, but patients also have some responsibilities during the period of treatment.Moreover, the participation of the patients? neighbourhood to the treatment period will increase success and the adherence.Key words: Hypertension, Adherence, Blood Pressure, Compliance

Bu araştırma, Kırklareli Devlet Hastanesi kardiyoloji polikliniğine başvurup ilk kez hipertansiyon tanısı alan hastaların tedavisine uyum düzeylerinin ve bunu etkileyen faktörlerin belirlenmesi amacıyla yapılmış bir izlem çalışmasıdır.Mart, Nisan, Mayıs 2007 tarihlerinde kardiyoloji polikliniğine başvuran ilk kez hipertansiyon tanısı alan ve çalışmaya katılma kriterlerini taşıyan 210 hasta çalışmaya alınmıştır. Araştırmada örneklem seçilmemiş tüm hastalara ulaşmak hedeflenmiştir. Tanıdan üç ay sonra hastalar izlem için kontrole çağrılmıştır. İzlemleri tamamlananların oranı %94'tür.Hipertansiyon tanısı en az bir haftadır tansiyonu yüksek seyreden polikliniğe başvurduğunda istirahat sonrası yapılan ölçümlerde tansiyonu normal değerlerin üstünde bulunanlara konulmuştur. Hipertansiyonun evrelenmesi JNC7 raporuna göre yapılmıştır. Demografik özellikler ve davranışa ait değişkenler ilk kez tanı alanlara başvurdukları sırada uygulanan, anket ile toplanmıştır. Tedavi uyumunu değerlendirmek için üçay sonra üçüncü bir anket uygulanmıştır. Uyum üç düzlemde ölçülmüştür; 1)İlaç uyumu, 2)İlaç dışı tedavi uyumu, 3)Kontrol uyumu.Analizlerde değişkenler arasındaki ilişkiyi belirlemek için,Ki-Kare,Paired-SamplesT ve McNemar testleri kullanılmıştırBu çalışmada ilaç uyumu %62 bulunmuştur. İlaç kullanmama nedenleri arasında ilk sırada?ilaç almayı unutma? gelmektedir (%73). Kontrol uyumu ilaç uyumundan daha yüksek bulunmuştur (%82). İlaç uyumu arttıkça kontrol uyumu da artmaktadır (p<0,001). Katılımcıların çoğu ilaç dışı tedavi önerilerini uyguladıklarını belirtmiştir. Üç ay sonunda katılımcıların tümünün BKİ'nde azalma olmuştur (p<0,001). Bu azalmanın ilaç ve kontrol uyumlu hastalarda daha anlamlı olduğu belirlenmiştir (p<0,05). Kan Basıncı (KB) kontrol oranı %82 olarak bulunmuş, KB kontrolü ile ilaç, kontrol uyumu arasında anlamlı bir ilişki belirlenmiştir (p<0,001).Hipertansiyon tedavisinde başarı, tedavi uyumu ile artmaktadır. Başarının sadece ilaç tedavisiyle sağlanamayacağı, tedavi sürecinde hastalarında üzerine düşen görevler olduğu hastalara anlatılmalıdır. Hasta yakınlarının tedavi sürecine katılması bunun sağlanmasına yardımcı olacaktır.
