Abstract

Er;YAG lazer ve konvansiyonel cerrahi ile yapılan osteotomilerde termal değişikliklerin taze sığır kemiğinde in vitro incelenmesi.Amaç: Er;YAG lazerin, konvansiyonel freze göre in vitro ortamda, taze sığır kemik bloklarında yapılan osteotomilerinde oluşan ısı farklılıklarının ölçülmesidir. Gereç ve Yöntem: Er;YAG lazer (AT Fidelis Plus Fotona Er;YAG, Nd;YAG lazer cihazı, Ljubljana, Slovenia) ve cerrahi piyasemen (NSK EX-6B, Japonya) kullanıldı. In vitro çalışma materyali olarak taze sığır kaburga kemikleri, ısı farklılığının ölçümü için Picolog USB TC-08 Thermocouple Data Logger, Pico Tech, China cihazı kullanıldı. Kemik bloklar üzerinde yüzeyden 2mm derinlikte kenara 2 cm mesafede olacak şekilde serum soğutması altında, 1 mm çapında tungsten karbid cerrahi rond frez ile yuvalar hazırlandı. Açılan yuvalara termokupl sensörleri yerleştirildi. Osteotomi işlemleri, bloklar üzerinde belirlenen hattan 2 mm mesafede yüzeyden derine inecek şekilde Er;YAG lazer, frez ve piezo cerrahi cihazları ile uygulandı ve oluşan ısı değişimleri sensörler aracılığıyla bilgisayara aktarıldı. Çalışma her üç cihaz içinde üç farklı ısıl ortamda; 12°C, 25°C(dış ortam ısısı) ve 32°C'de uygulandı.Bulgular: 32°C, 12°C ve dış ortam (25°C) sıcaklıklarında; Er;YAG lazer cihazı 12°C ısıl ortamda en az ısıl farklılığa sebep olurken (0,17±0,10°C), en yüksek ısıl farklılık dış ortam sıcaklığında oluştu. (1,01±0,25°C). Er;YAG lazer, piezo cihazına göre daha az, piezo cihazı ise freze oranla daha az ısıl farklılık oluşturdu. P(Lazer-Frez)<0,001 P(Lazer-Piezo)=0,955 P(Frez-Piezo)<0,001Sonuçlar: Çalışmamız sonucunda elde edilen değerlere göre; Er;YAG lazer ile uygulanan osteotomi işlemlerinde her üç ısıl ortam içinde termal zarar meydana gelmediği ve konvansiyonel frez ve piezo cerrahi cihazlarına oranla daha az termal değişikliklere sebep olduğu ortaya çıkmıştır. In vitro Investigation of thermal changes in the fresh bovine rib bone osteotomy performed with the Er;YAG laser and conventional drill. Aim; Comparing the temperature changes between Er;YAG laser and conventionel surgical drill osteotomy on fresh bovine rib bone under in vitro conditions.Material and method; Er;YAG laser device (AT Fidelis Plus Fotona Er;YAG, Nd;YAG laser cihazı, Ljubljana, Slovenia) and conventional handpiece with 1mm round tungsten carbid surgical drill used for the procedure. As an osteotomy material fresh bovine rib bone blocks were chosen. Temperature changes were measured with a thermocouple device, Picolog USB TC-08 Thermocouple Data Logger, Pico Tech, China. Each of them is in 1cmx2cmx5cm sizes totally 142 bone blocks were prepared. On each block, 1mm diameter hole was opened with surgical round drill 2 mm from surface and 2 cm to the edge under saline solution for placing thermocouple sensors. Then 2 mm away from the sensors vertical osteotomies were performed with surgical drills, Er;YAG laser device and piezosurgery device. The temperature datas were measured and sent to the computer. Each device was studied under 12°C, 32°C water tanks and 25°C room temperature.Results; Er;YAG laser generated significantly lower heat (0,17±0,10°C) than the conventional technique and piezosurgery at 12°C water tank and the most temperature increase was occured at 25°C (1,01±0,25°C). P(Laser-Drill)<0,001Conclusion; As a result of our study we found that Er;YAG laser osteotomy is a safe method on corticocancellous bone blocks with no thermal damage and also it causes less temperature increase than surgical drills and piezosurgery device. But only disadvantage of this device is its speed. Its slower than both methods.Key words; Er;YAG laser, drill, piezosurgery, osteotomy, thermocouple
