Samproduksjon i psykisk helsevern for barn og unge: En studie av pasientsentrering gjennom digitale helseinnovasjoner

Abstract

The psychiatric part of the health care system in Norway is experiencing increasing pressure, where the covid-19 pandemic has lead to a further increase. For children in mental health care, it is particularly diffcult to gain insight into the complex conditions surrounding their own treatment. The lack of understanding leaves the child insecure about his or her own state of health and delivery process, which in turn involves demanding conditions during assessment and treatment. At the same time, a lack of insight into one's own illness and the implications this has will lead to a distancing from one's own life, and can have major consequences for the child's further life. Therefore, the need for patient centering and involvement is particularly signi cant for child psychiatry. Digitization of public services is generally regarded as a signi cant facilitator of customer involvement in delivery processes. Through the use of technological solutions and an approach to co-production, services are moving towards individualized offers and processes. In order for the health service to ensure a patient-centered health model, it is therefore necessary to prioritize digital health solutions, as well as the inclusion of patients through a seamless delivery. Consequently, this study is based on co-production and digital health innovations to address the challenges in mental health care for children and young people. The overall issue thus concerns securing children under psychiatric treatment's ownership of their own state of health and delivery process, through digitization and patient involvement. To meet this, a qualitative case study has been carried out against a psychiatric ward for children under Haukeland University Hospital - Barneposten. The results from the study have led to the establishment of the company Adfectus, which through a digital health platform wants to improve children in the patient role's prerequisites for taking an active part in the design of their own health delivery. The empirical ndings show a lack of patient information and involvement. This is largely due to the extensive use of linguistic and advanced dissemination methods. On the other hand, the study illustrates the signi cant effect of using digital and visual tools, and thus digital health innovations have great potential in child psychiatry. In addition, focus has been placed on what changes are required in the delivery process for an appropriate implementation.

Den psykiatriske delen av helsevesenet i Norge opplever stadig mer press, hvor covid- 19-pandemien har økt det ytterligere. For barn under psykisk helsevern er det særlig vanskelig å oppnå innsikt i de komplekse forholdene rundt egen behandling. Den manglende forståelsen gjør barnet utrygg på egen helsetilstand og leveranseprosess, som igjen innebærer krevende forhold under utredning og behandling. Samtidig vil manglende innblikk i egen sykdom og implikasjonene dette gir føre til en distansering fra eget liv, og kan få store følger for barnets videre liv. Derfor er behovet for pasientsentrering og - involvering særlig betydelig for barnepsykiatrien. Digitalisering av offentlige tjenester blir generelt ansett som en betydelig fasilitator for involvering av kunder i leveranseprosesser. Gjennom bruk av teknologiske løsninger og en tilnærming til samproduksjon beveger tjenester seg over mot individualiserte tilbud og prosesser. For at helsevesenet skal sikre en pasientsentrert helsemodell blir det derfor nødvendig å prioritere digitale helseløsninger, samt inkludering av pasienter gjennom en helhetlig og sømløs leveranse. Følgelig tar denne studien utgangspunkt i samproduksjon og digitale helseinnovasjoner for å adressere utfordringene i psykisk helsevern for barn og unge. Den overordnede problemstillingen omhandler dermed sikring av barn under psykiatrisk behandlings eierskap til egen helsetilstand og leveranseprosess, gjennom digitalisering og pasientinvolvering. For å møte dette har det blitt gjennomført en kvalitativ casestudie mot en psykiatrisk sengepost for barn under Haukeland Universitetssykehus - Barneposten. Resultatene fra studien har ført til opprettelsen av bedriften Adfectus, som gjennom en digital helseplattform ønsker å bedre barn i pasientrollens forutsetninger for å ta en aktiv del i utformingen av egen helseleveranse. De empiriske funnene viser en mangelfull pasientopplysning og -involvering. Dette mye på grunn av en omfattende bruk av språklige og avanserte formidlingsmetoder. Det er derimot gjennom studien illustrert betydelig effekt ved bruk av digitale og visuelle virkemidler, og dermed har digitale helseinnovasjoner et stort potensiale i barnepsykiatrien. I tillegg er det lagt fokus på hvilke endringer som kreves i leveranseprosessen for en hensiktsmessig implementering.
