Factores de riesgo psicosocial del TDAH

Abstract

El objetivo de la tesis es analizar el impacto de factores de riesgo psicosocial en la evolución del trastorno por deficit de atención e hiperactividad (TDAH) en una muestra clínica de niños y adolescentes tratados en centros de atención especializada en salud mental. En el primer artículo de la tesis se analizó la asociación entre factores de riesgo psicosocial recogidos en la clasificación de enfermedades mentales de la OMS (CIE-10) y en el segundo estudio se analizó el efecto de un factor específico: la psicopatología paterna y materna y los síntomas de TDAH en padres de niños en seguimiento por TDAH. En el primero se encontraron asociaciones entre factores de riesgo familiar y mala evolución clínica del TDAH en los hijos, mientras que en el segundo estudio se encontraron elevadas tasas de psicopatología y síntomas TDAH en padres y madres y asociaciones entre los antecedentes de psicopatología materna y mala evolución sintomática de los hijos. Se discutieron las implicaciones para la práctica clínica que tienen estos resultados, básicamente el diseño de protocolos específicos para pacientes con mala evolución clínica con factores de riesgo asociados.

L'objectiu de la tesi és analitzar l'impacte de factors de risc psicosocial en l'evolució del trastorn per dèficit d'atenció i hiperactivitat (TDAH) en una mostra clínica de nens i adolescents tractats en centres d'atenció especialitzada. En el primer article de la tesi s'analitza l'associació entre factors de risc psicosocial recollits en la classificació de malalties mentals de l'OMS (CIE-10) i en el segon estudi s'analitza l'efecte d'un factor específic, la psicopatologia paterna i materna i els símptomes de TDAH en pares de nens en seguiment per TDAH. En el primer estudi es troben associacions entre factors de risc familiar i la mala evolució clínica del TDAH en els fills. En el segon estudi es troben elevades taxes de psicopatologia i símptomes TDAH en pares i mares i associacions entre els antecedents de psicopatologia materna i mala evolució simptomàtica dels fills. Es discuteixen les implicacions per a la pràctica clínica que tenen aquests resultats, bàsicament el disseny de protocols específics per a pacients amb mala evolució clínica amb factors de risc associats.

The aim of the thesis is to analyze the impact of psychosocial risk factors in the development of attention deficit and hyperactivity disorder (ADHD) in a clinical sample of children and adolescents treated in Specialized Mental Health Centers. In the first article of the thesis we found an association between psychosocial risk factors listed in the classification of mental diseases WHO (ICD-10) and in the second one we analyzed the effect of a specific factor: parental psychopathology and ADHD symptoms in parents of children treated for ADHD. In the first study we have found associations between family risk factors and poor clinical outcome of ADHD children. In the second one we have found high rates of psychopathology and ADHD symptoms in parents of children with ADHD and also associations between maternal history of psychopathology and greatest symptoms severity of ADHD children. We discuss the implications for clinical practice of these results, basically the design of specific protocols for patients with poor clinical outcomes with associated risk factors.
