Braquiterapia epiescleral en el melanoma de coroides

Abstract

La braquiterapia oftálmica supone una alternativa de tratamiento conservador a la enucleación en el melanoma de coroides que , en muchos casos, permite mantener tanto el órgano como la visión. Objetivo: Analizar la experiencia de nuestro centro de forma retrospectiva y nuestros resultados en cuanto a toxicidad, control local y supervivencia Material y Métodos: Desde 1996 al 2004 se han tratado con placa epiescleral con semillas de I-125 120 pacientes afectos de melanoma de coroides; de ellos 50 eran hombres y 70 mujeres. Mediana de edad 59 años. Localización:60 en ojo dcho y 58 en ojo izdo. 3 pacientes fueron estadiados como COMS 1, 87 pacientes COMS 2; 24 COMS 3 y 9 COMS indeterminado. Seguimiento mínimo: 1 año, seguimiento máximo: 8.4 años Resultados: La supervivencia global a 5 años ha sido de 83,92% y a 8 años de 73,38%. La supervivencia especifica a 5 años 85,71% y a 8 años ha sido la misma. El control local a 5 años ha sido de 88.25% y a 8 años el mismo. La supervivencia libre de metástasis ha sido de 79,49% a 5 años y 72,73% a 8 años. En cuanto a toxicidad encontramos que el principal efecto secundario fue la producción de cataratas en un 38% de pacientes, seguida de la retinopatía por radiación en un 7,5 % de casos, desprendimiento de retina en un 4,17%,maculopatía en el 4,1%, hemorragia vítrea en un 3, 3% y estrabismo en el 0.83%. Conclusión: El tratamiento del melanoma de coroides con placa epiescleral con semillas de I-125 ofrece la posibilidad de conservar el ojo con un perfil de toxicidad relativamente bien tolerado por el paciente y un control de la enfermedad similar a otras técnicas.
