Manejo de la inflamación postoperatoria y el dellen corneal tras cirugía de pterigion mediante el uso de NETDEX

Abstract

El pterigion es una lesión degenerativa fibrovascular subepitelial de la conjuntiva bulbar que se extiende en forma triangular sobre la córnea1,2. Al igual que la pinguécula puede ser reactivo a la exposición solar y otros factores de riesgo como el ojo seco crónico4. A nivel histológico muestra cambios degenerativos elastósicos en el colágeno estromal subepitelial vascularizado3-5. En referencia al cuadro clínico, la mayoría de las pequeñas lesiones suelen ser asintomáticas. Puede haber síntomas de irritación y sensación de cuerpo extraño, así como de disminución de visión si tapa el eje visual o genera un astigmatismo inducido. Las lesiones extensas pueden asociarse a fibrosis subconjuntival que alcance los fondos de saco y cause restricción de los movimientos oculares2,7. El tratamiento médico del pterigion se basa en el uso de lubricantes tópicos o una pauta corta de corticoides si hay signos de inflamación asociados. Uno de los procedimientos quirúrgicos más indicados en la actualidad para su tratamiento, es la resección y posterior recubrimiento con un autoinjerto conjuntival, utilizando como donante la zona paralímbica o temporal superior. La fijación del injerto con adhesivo tisular acorta la duración de la intervención, y reduce la sintomatología irritativa postoperatoria en comparación al empleo de suturas. Como tratamiento postquirúrgico se suele pautar lágrima artificial asociado al uso de corticoides tópicos.
