Accidentes en la infancia. Resultados del Servicio de Urgencias del Hospital La Fe

Abstract

Objetivo: Analizar la epidemiología de los accidentes infantiles atendidos en un Servicio de Urgencias hospitalario con vistas a la prevención. Método: Estudio descriptivo, retrospectivo con componente analítico. Hospital Infantil La Fe, Valencia, que cubre una población pediátrica de 164.127 niños de 0-14 años. Pacientes atendidos en el Servicio de Urgencias y remitidos a su domicilio por un accidente, durante 1993. Resultados: Se atendieron 50.726 pacientes, 10.800 (21,3%) por accidente. Incidencia anual de 5.959,4/100.000. Predominio de varones (57,1%)(p = 0,000) y de 0-4 años (41,3%)(p = 0,000). Edad (media*DE) 74,3 ± 41,9 meses. Significación (p = 0,001) de edad y sexo (varones 75,4 ± 41,6 meses; niñas: 72,7 ± 42,3). Máximo lunes (15,6%)(p = 0,000) y junio (10,5%)(p = 0,000). Principales diagnósticos: contusiones (23,5%), heridas (19,3%) y fracturas (13,6%); en el 1% la exploración fue normal. Acudieron al hospital espontáneamente el 77,2% y pertenecían al sector sanitario La Fe el 90,6%. Significación del diagnóstico y sexo (p = 0,000), edad (p = 0,000), número de habitantes de la población de residencia (p = 0,000), provincia (p = 0,033), hora de llegada (p = 0,000), día de la semana (p = 0,000), mes (p = 0,000), procedencia (espontánea o con atención médica previa)(p = 0,000) y sector sanitario (p = 0,000). Conclusiones: Se deberían realizar campañas de educación sanitaria para disminuir la frecuencia de accidentes en la infancia, sobre todo en varones y edades 0-4 años y mejorar la utilización de los servicios sanitarios accediendo al nivel adecuado según gravedad y urgencia del paciente lesionado.
