Consideraciones epidemiológicas sobre criptosporidiosis

Abstract

El presente trabajo constituye una revisión de los aspectos epidemiológicos más relevantes de la criptosporidiosis. El interés sanitario de esta enfermedad, causada por protozoos Apicomplexa, se debe a su mayor incidencia en pacientes con alteraciones inmunológicas, particularmente en afectos de SIDA. La principal vía de infección es la oral, ya sea por transmisión directa o indirecta. La frecuencia de infección es variable, siendo los menores más susceptibles. El estado inmunológico del hospedador determina el curso de la enfermedad. En individuos inmunocompetentes, la infección cursa bien de una manera asintomática, bien con una diarrea moderada y profusa que revierte espontáneamente. En inmunodeficientes se produce una severa y prolongada diarrea con destrucción de enterocitos. En el diagnóstico e identificación del parásito, las técnicas de tinción de Ziehl-Neelsen modificada y las inmunofluorescencias directa e indirecta con anticuerpos nomoclonales presentan los mejores resultados. La búsqueda de un tratamiento etiológico ha sido decepcionante. La profilaxis debe encaminarse a incrementar las condiciones higiénicas.
