Abstract

A elaboração do presente documento decorre no âmbito da unidade curricular “Estágio Final e Relatório”, no decorrer do Curso de Mestrado em Enfermagem Médico-Cirúrgica do Instituto de Ciências da Saúde (Porto), da Universidade Católica Portuguesa. O relatório é intitulado “Ser Enfermeiro Especialista: O Desafio do Percurso” e tem como objetivo evidenciar a aquisição de competências comuns e específicas do enfermeiro especialista em Enfermagem Médico­Cirúrgica, durante o estágio em contexto de cuidados intensivos, realizado numa Unidade de Cuidados Intensivos Polivalente e o ensino clínico no Grupo de Coordenação Local – Programa de Prevenção e Controlo de Infeção e Resistência a Antimicrobianos, ambos decorridos num hospital, no norte do país. Na construção deste documento foi privilegiada a metodologia descritiva e reflexiva para abordar as experiências vividas nos dois contextos de prática clínica que levaram à aquisição de competências de enfermagem especializada, pela diferenciação de cada contexto. No decurso do período de estágio, foram identificadas necessidades formativas e desenvolvidas estratégias para as colmatar, nomeadamente, na área da prevenção e controlo de infeção foi elaborada uma formação sobre “Feixe de Intervenções para a Prevenção da Infeção Urinária associada a Cateter Vesical”, que pretendeu divulgar as boas práticas, nas equipas de enfermagem do hospital, baseadas na mais recente evidência científica. Para além do contributo de melhoria para o serviço, foram realizadas pesquisas bibliográficas e adotada uma postura proativa, que permitiram o desenvolvimento de competências comuns do enfermeiro especialista, nos domínios da responsabilidade profissional, ética e legal; na melhoria contínua da qualidade, através da garantia de um papel dinamizador no desenvolvimento e suporte das iniciativas estratégicas institucionais na área da governação clínica; na gestão dos cuidados, otimizando a resposta da equipa e a articulação na equipa de saúde e de adaptação da liderança e da gestão de recursos às situações e ao contexto, objetivando a garantia da qualidade e segurança dos cuidados prestados; e no domínio do desenvolvimento das aprendizagens profissionais, desenvolvendo o autoconhecimento e assertividade, baseando a tomada de decisão especializada em evidência científica. De realçar, a aquisição de competências específicas do enfermeiro especialista em Enfermagem Médico­Cirúrgica, no que concerne: ao cuidado da pessoa, família/cuidador a vivenciar processos complexos de doença crítica e/ou falência orgânica; à dinamização da resposta em situações de emergência, exceção e catástrofe e à maximização da prevenção, intervenção e controlo de infeção e de resistência a antimicrobianos perante a pessoa em situação crítica e/ou falência orgânica, face à complexidade da situação e à necessidade de respostas em tempo útil e adequadas.

The elaboration of this document comes within the scope of the curricular unit “Final Internship and Report”, during the Master's Degree in Medical­Surgical Nursing at the Institute of Health Sciences (Porto) of the Universidade Católica Portuguesa. This report is entitled “Being a Specialist Nurse: The Challenge of the Path” and aims to highlight the acquisition of common and specific skills of the specialist nurse in Medical Surgical Nursing, during the internship in the context of intensive care, carried out in a Polyvalent Intensive Care Unit and clinical teaching in the Local Coordination Group –Infection Prevention and Control Program and Antimicrobial Resistance, both held in a hospital in the north of the country. In the construction of this document, a descriptive and reflective methodology was privileged to address the experiences lived in the two contexts of clinical practice that led to the acquisition of specialized nursing skills, by differentiating each context. During the internship period, training needs were identified and strategies developed to address them, namely in the area of infection prevention and control, training was prepared on the “Bundle for the Prevention of Urinary Infection associated with Bladder Catheter”, which intended to disseminate good practices in the hospital’s nursing teams, based on the most recent scientific evidence. In addition to contributing to the improvement of the service, bibliographic research was carried out and a proactive attitude was adopted, which allowed me to develop common skills of specialist nurses in the fields of professional, ethical and legal responsibility; in the continuous improvement of quality, by guaranteeing a dynamic role in the development and support of institutional strategic initiatives in the area of clinical governance; in care management, optimizing the team's response and articulation in the health team and adaptation of leadership and resource management to situations and context, with the purpose to assuring the quality and safety of the care provided; and in the field of professional learning development, developing self-knowledge and assertiveness, basing specialized decision-making on scientific evidence. It should be highlighted the acquisition of specific skills of nurses specializing in Medical­Surgical Nursing, with regard to: caring for the person, family/caregiver experiencing complex processes of critical illness and/or organ failure; to streamline the response in emergency, exception and catastrophe situations and to maximizing prevention, intervention and control of infection and antimicrobial resistance for people in critical situations and/or organ failure, given the complexity of the situation and the need for responses in a timely and appropriate way.
