Abstract

Introdução: Os nódulos da tiroide são muito prevalentes na população. Sendo uma pequena percentagem correspondente a neoplasias malignas da tiroide, torna-se fundamental dispor de um sistema de classificação que permita definir a melhor abordagem a cada lesão. Para responder a este desafio, foi definido o Thyroid Imaging Reporting and Data System (TI-RADS).Objetivos: Descrever o TI-RADS de um ponto de vista prático, analisar os seus contributos, apontar eventuais limitações e apresentar as principais técnicas que visam otimizar a sua aplicação.Resultados: O TI-RADS aborda 6 características dos nódulos tiroideus para decidir quanto à necessidade de citologia aspirativa com agulha fina e à periodicidade do follow-up. Apesar de ser utilizado em vários centros, são apontadas falhas significativas no TI-RADS quando comparado com outras guidelines. Com vista a potenciar o TI-RADS, alguns autores propõem a combinação com outras técnicas, nomeadamente, o uso de contraste, a elastografia e um sistema semiautomático de inteligência artificial.Conclusão: Pela análise da informação disponível na literatura e apesar das dúvidas levantadas por alguns autores acerca das vantagens que poderá trazer relativamente às guidelines anteriores, compreende-se que o TI RADS é útil na abordagem dos doentes com nódulos da tiroide, permitindo ter um método sistemático e objetivo de avaliação destas lesões. Parece reduzir significativamente o número de citologias por punção aspirativa desnecessárias, sendo benéfico em termos de custo para um sistema de saúde. Ainda assim parece-nos pertinente a sua aplicação cautelosa associada a estudos comparativos entre o TI-RADS e outros sistemas de classificação de nódulos tiroideus.

Introduction: Thyroid nodules have become a common finding. Since a small percentage are malignant thyroid neoplasms, it is vital to build a classification system that allows physicians to decide the best approach for each lesion. Thyroid Imaging Reporting and Data System (TI-RADS) was created to take on this challenge.Objectives: Describe TI-RADS from a practical perspective, analyze its main contributes, expose some potential flaws, and display the main procedures that aim to optimize TI-RADS application.Results: TI-RADS addresses 6 features in the thyroid nodules to plan on the need for fine-needle aspiration and follow-up periodicity. Despite being used in several centers, important flaws in TI-RADS are pointed out when compared to other guidelines. To enhance TI-RADS, some authors suggest a combination with other techniques, namely the use of contrast, elastography and a semiautomatic artificial intelligence system.Conclusion: Considering the data available in the literature and despite the doubt raised by some authors about the benefits it may bring in relation to the previous guidelines, it is accepted that TI RADS is useful in the approach of patients with thyroid nodules, allowing a systematic and objective method of evaluating these lesions. It seems to significantly reduce the amount of cytology per fine-needle aspiration, which brings benefits in terms of cost for a health system. Even so, being relatively recent, the cautious application seems to be pertinent, requiring comparative studies between TI-RADS and other thyroid nodule classification systems.
