Abstract

Introdução: A surdez súbita sensorio-neural é uma emergência otológica. Atualmente é possível identificar a sua etiologia apenas em cerca de 10% dos casos. A hemorragia intralabiríntica é uma causa rara de surdez súbita, cursando frequentemente com sintomatologia vestibular concomitante. O objetivo deste trabalho é analisar a literatura relativa a hemorragia intrabiríntica, para melhor compreender a sua fisiopatologia, bem como a abordagem clínica desta entidade.Materiais e Métodos: A pesquisa bibliográfica foi realizada na plataforma PubMed de acordo com os critérios de inclusão e de exclusão predefinidos. Foram selecionados estudos prospetivos e retrospetivos, bem como relatos de casos clínicos que envolviam explicitamente hemorragia intralabiríntica publicados entre 2015 e 2020. A partir da análise integral dos artigos selecionados e de outros artigos igualmente relevantes obtidos por referência cruzada, foi elaborada a presente revisão sistemática.Resultados: Foram incluídos nesta revisão 13 artigos no total. Evidenciaram-se dados relevantes sobre manifestações clínicas, fatores de risco, exames complementares de diagnóstico, prognóstico e tratamento subjacentes à hemorragia intralabiríntica. A prevalência de sintomas vestibulares, em particular a vertigem, revelou-se significativamente superior nos doentes com surdez súbita secundária a hemorragia intralabiríntica, comparando com doentes com surdez súbita secundária a outras etiologias. Vários doentes com hemorragia intralabiríntica apresentaram história de anticoagulação, mas verificou-se não ser condição sine qua non. Constatou-se ainda que os doentes com hemorragia intralabiríntica tendem a apresentar uma perda auditiva mais profunda, bem como um pior prognóstico auditivo, comparando com doentes com surdez súbita não secundária a esta entidade.Conclusão: Há ainda lacunas evidentes no que concerne à fisiopatologia da hemorragia intralabiríntica que se traduzem em dificuldades na abordagem clínica destes doentes. A terapêutica atual não é eficaz e ainda não existem critérios de prognóstico bem definidos. Assim, a realização de novos estudos com maiores populações e com períodos de follow-up mais prolongados revela-se fundamental.

Introduction: Sudden sensorineural hearing loss is an otological emergency. Currently, it is possible to identify its etiology in only about 10% of cases. Intralabyrinthine hemorrhage is a rare cause of sudden deafness, often occurring with concomitant vestibular symptoms. The objective of this study is to analyze the literature related to the intrabyrinthine hemorrhage in order to better understand its pathophysiology, as well as the clinical approach of this entity.Materials and Methods: The bibliographic research for the elaboration of this article was obtained through the platform PubMed according to the predefined inclusion and exclusion criteria. Prospective and retrospective studies and clinical case reports which explicitly involved intralabyrinthine hemorrhage published between 2015 and 2020, were selected. Based on the full analysis of the selected articles and other equally relevant articles obtained by cross-reference, the present systematic review was carried out. Results: A total of 13 articles were included in this review. Relevant data on clinical manifestations, risk factors, complementary diagnostic tests, prognosis and treatment underlying the intralabyrinthine hemorrhage were evidenced. The prevalence of vestibular symptoms, in particular vertigo, was significantly higher in patients with sudden hearing loss secondary to intralabyrinthine hemorrhage, comparing with patients with sudden hearing loss secondary to other etiologies. Several patients with intralabyrinthine hemorrhage had history of anticoagulation, although it is not a sine qua non condition. In addiction, it was found that patients with intralabyrinthine hemorrhage tend to have a deeper hearing loss, as well as a worse hearing prognosis, comparing with patients with sudden hearing loss not secondary to this entity.Conclusion: There are still evident gaps related to the pathophysiology of intralabyrinthine hemorrhage that translate into difficulties in the clinical approach of these patients. Current therapy is not effective and there are still no well-defined prognostic criteria. Thus, new studies with larger populations and longer follow-up periods are essential.
