Abstract

A hiperidrose define-se como a produção excessiva de suor pelas glândulas sudoríparas écrinas, relativamente às necessidades fisiológicas de manutenção da homeostase térmica. É classificada etiologicamente em primária (idiopática) ou secundária, quando surge no contexto de condições médicas como no caso de infeções, patologia endócrina, neurológica e oncológica, ou por iatrogenia medicamentosa. Pode apresentar-se de forma focal ou generalizada, sendo a primeira mais associada à hiperidrose primária e a segunda à forma secundária da doença. O risco para patologia dermatológica concomitante, como verrugas vulgares, é superior em doentes com hiperidrose. Esta é uma doença comum, sendo pouco valorizada, e frequentemente subdiagnosticada em idade pediátrica. Pelo desconforto e ansiedade que causa nas atividades de vida diárias e interações sociais, pelo importante impacto na qualidade de vida, pode ter consequências no desenvolvimento psicossocial das crianças e adolescentes. O diagnóstico precoce e a terapêutica adequada são essenciais no controlo dos sintomas, diminuindo a morbilidade e aumentando a autoestima dos doentes. As opções terapêuticas atuais passam por antitranspirantes tópicos, iontoforese, toxina botulínica, medicação anticolinérgica, dispositivos médicos e, em última linha, a possibilidade de intervenção cirúrgica. O presente trabalho pretende fazer uma revisão bibliográfica, focando-se no diagnóstico e tratamento da hiperidrose em crianças, bem como nas novas opções terapêuticas na abordagem desta patologia tão frequente e com tão grande impacto social, mas frequentemente desvalorizada na prática clínica.

Hyperhidrosis is defined as an excessive sweat production by the eccrine sweat glands relative to the physiological needs on maintaining thermal homeostasis.It is etiologically classified as primary (idiopathic) or secondary when it arises in the context of medical conditions such as infections, endocrine, neurological and oncological pathology, or by drug iatrogenia. It may be focal or generalized, the first more commonly associated with primary hyperhidrosis and the second with the secondary form of the disease. The risk for concomitant dermatological pathology, such as common warts, is higher in patients with hyperhidrosis.This is a common and undervalued disease being oftenly underdiagnosed in children. Because of the discomfort and anxiety it causes in daily life activities and social interactions, it has an important impact on quality of life, and can have consequences on the psychosocial development of children and adolescents.Early diagnosis and appropriate therapy are essential in symptom control, reducing morbidity and increasing patient’s self-esteem.Current therapeutic options include topical antiperspirants, iontophoresis, botulinum toxin, anticholinergic medication, emerging medical devices and, ultimately, the possibility of surgical intervention.The present work intends to make a bibliographical review, focusing on the diagnosis and treatment of hyperhidrosis in children, as well as the new therapeutic options in the approach of this pathology so frequent and with such a great social impact, but often devalued in the clinical practice.
