Abstract

IntroduçãoA perceção dos doentes quanto ao consentimento informado (CI) não está bem estabelecida sendo, frequentemente, considerado como um mero ato administrativo. Por outro lado, as questões médico-legais que envolvem o CI são uma problemática atual. O objetivo principal foi avaliar o grau de informação dos doentes em relação ao CI, na perspetiva dos próprios.MétodosEstudo prospetivo, realizado em dois centros hospitalares, um deles com acreditação. Aplicação de questionários consecutivos, preenchidos pelo próprio doente, após a realização de exame endoscópico.ResultadoAmostra constituída por 171 doentes, na sua maioria do sexo masculino (65,1%), e, dos quais, 84,2% com idade igual ou superior a 50 anos. A maioria dos exames foi realizada em contexto eletivo (94,7%).A maioria dos inquiridos foi informada do motivo da realização do procedimento (97,6%) e inteirada do benefício, risco e complicações do mesmo pelo médico prescritor (96,5%). A generalidade dos doentes ficou esclarecida com a informação do CI Os aspetos apontados como menos valorizados no CI foram os riscos/complicações (34,4%), procedimento (29,8%) e benefícios (22,9%) sendo que 31,7% dos doentes procedeu a pesquisa adicional sobre o procedimento, maioritariamente através da internet. A quase totalidade (98,2%) dos doentes assinou o CI referindo ter sido disponibilizado tempo suficiente para reflexão (95,2%) e 58,5% dos doentes ficou com uma cópia do CI assinado. O médico executante confirmou que o doente estava devidamente informado em 94,6% casos fornecendo informações no final em 94,5%. Comparando os dois centros verificaram-se diferenças estatisticamente significativas na entrega do CI (p= 0,0001), na confirmação do esclarecimento pelo médico executante (p= 0.011), no fornecimento de informações após o exame (p= 0,012) e no grau de esclarecimento (p= 0,014), tendo-se obtido mais respostas negativas no centro sem acreditação.ConclusãoA maioria dos doentes sente-se informado com o CI, embora se tenha verificado que uma proporção substancial refere lacunas de informação nomeadamente no que diz respeito aos riscos/complicações dos procedimentos.

IntroductionThe patient´s perception of informed consent (IC) is not well established and is very often regarded as an administrative act. On the other hand, the medical-legal issues involving IC are a current problem. The main goal was to assess the degree of patient´s information about IC from their perspective.MethodsA prospective study was carried out in two hospital centres, one of them with accreditation. There was an application of questionnaires, fulfilled by the patient, after the endoscopic examination.ResultsThe sample consisted of 171 patients, mostly males (65,1), and of which 84.2% were 50 years old or older. Most of the tests were performed in an elective context (94.7%).The majority of respondents were informed of the procedure´s purpose (97.6%) and were made aware of the benefit, risk and complications of it by the prescribing physician (96.5%). Most of the patients were enlightened by the information of IC (90.5%). The aspects pointed out as less valued in IC were the risks/complications (34.4%), procedure (29.8%) and benefits (22.9%) and 31.7% of patients conducted additional research on the procedure, mostly over the internet. The majority (98.2%) of patients signed the IC, saying that they had enough time for reflection (95.2%) and 58.5% of patients were left with a copy of the signed IC. The executing physician confirmed that the patient was properly informed in 94.6% cases providing information at the end in 94.5% cases.Comparing the two centres, statistically significant differences were found in the delivery of IC´s copy (p= 0.0001), the confirming of the explanation by the executing physician (p= 0.011), in the delivery of information after the examination (p= 0.012) and in the degree of clarification (p= 0.014). More negative responses were obtained in the center without accreditation.ConclusionMost patients feel informed with IC, although it has been found that a substantial proportion mentions information gaps particularly regarding the risks/complications of procedures.
