Abstract

Disciplina afim: Psiquiatria e Saúde Mental O presente trabalho envolveu o estudo comparativo de dois grupos consecutivos de doentes, frequentando a Consulta de Reumatologia dos HUC, com os diagnósticos de fibromialgia (FM) e artrite reumatóide (AR). Foi adoptada uma dupla metodologia de investigação, de natureza quantitativa e qualitativa. Foram administrados diversos instrumentos psicométricos e um questionário de auto-registo a ambas as amostras de doentes, em dois tempos separados por um intervalo de seis meses. Os instrumentos psicométricos escolhidos avaliaram dimensões relacionadas com a presença de sintomatologia depressiva, psicopatologia geral, comportamento de doença anormal, amplificação somato-sensorial, saúde geral, estilo atribucional, e afectividade negativa. A abordagem qualitativa, por sua vez consistiu na realização de duas entrevistas semi-estruturadas, efectuadas com uma subamostra de doentes FM, seleccionados com base em critérios de riqueza de informação. A primeira destas entrevistas centrou-se na fenomenologia dos sintomas e respectivo impacto, nos espaços familiar e social. A segunda privilegiou as histórias de vida. Foram ainda feitas entrevistas, curtas, junto dos cônjuges. As conclusões do estudo obedeceram a um duplo formato, transversal e longitudinal. Ambas as perspectivas se revelaram compatíveis com o processo de somatização. A fibromialgia, independentemente dos putativos mecanismos fisiopatológicos que determinam a sua génese, ou acompanham a sua evolução temporal, pode ser entendida à luz dos processos psicológicos e sociais que caracterizam os processos de somatização e comportamento de doença.
