MULTIMORBILIDADE: ATIVIDADES DE VIDA DIÁRIA E NÍVEL SOCIOECONÓMICO, NO CENTRO DE PORTUGAL

Abstract

Introdução: A multimorbilidade (MM), presença de duas ou mais doenças crónicas, é cada vez mais um desafio para os médicos de família (MF). A prevalência da MM parece aumentar com a idade, pelo que os idosos são um grupo particularmente vulnerável. Objetivos: Avaliar a proporção de MM nos idosos em função da localização geográfica, idade e sexo, bem como perceber o seu impacto na realização das atividades de vida diária (AVD) e se o nível socioeconómico tem alguma influência na prevalência de MM nos idosos,em Portugal.Métodos: Estudo de dados quanto a idade, sexo, número de códigos ICPC2 e Índices de Barthel e de Graffar dos processos clínicos de todos os idosos inscritos em Centros de Saúde da ARS do Centro, dos 65 aos 99 anos, totalizando 190025 idosos, com uma média de idades de 80,16±8,03 anos. O Índice de Barthel foi utilizado para avaliar a dependência nas AVD e o Índice de Graffar o nível socioeconómico.Resultados: Os dados revelaram uma prevalência de 99,9% de MM nos idosos. Destes, 52,9% são do sexo feminino e a maioria estava no grupo etário 76-85 anos (39,1%). Cada idoso do sexo masculino apresenta em média 8,67 problemas e do sexo feminino 9,51 problemas. O preenchimento dos Índices de Barthel e de Graffar é baixo, no Índice de Barthel é de 7,7% e no Índice de Graffar 5,2%. Os idosos sem MM apresentam maiores taxas de dependência do que os doentes com MM (a dependência total com 7,4% vs 6,2% e a dependência grave 20,1% vs 9,6%). Os idosos com MM encontram-se principalmente em níveis socioeconómicos mais baixos, nomeadamente no médio (55,3% vs 27,5%) e no baixo(31,8% vs 19,6%), enquanto os idosos sem MM encontram-se sobretudo nos níveis socioeconómicos altos, nomeadamente no médio alto (35,3%) e no alto (17,6%).Conclusões: Maior MM está associada a classes socioeconómicas mais baixas. A dependência nas AVD parece não estar relacionada com a MM, segundo o Índice de Barthel. A abordagem dos MF nas consultas dos idosos deve ser a da MM. Estes doentes necessitam de uma atenção especial, pelos contextos social e económico, devido ao seu tratamento ter de abranger várias doenças crónicas em simultâneo, com riscos associados à polifarmacoterapia. Parece ser baixo o preenchimento dos Índices de Barthel e Graffar.

Introduction: Multimorbidity (MM), the presence of two or more chronic diseases, is increasingly a challenge for general practitioners (GPs). The prevalence of MM seems to increase with age, so the elderly are a particularly vulnerable group.Objectives: To evaluate the proportion of MM in the elderly according to geographic location, age and sex, as well as to understand its impact on activities of daily living (ADL)and whether the socioeconomic level has any influence on the prevalence of MM in the elderly, in Portugal.Methods: Data study on the age, sex, number of ICPC2 codes and Barthel and Graffar Indexes of the clinical processes of all the elderly enrolled in the health centers of Center ARS, between the ages of 65 to 99 years old, totaling 190025 elderly, with an average age of 80.16±8.03 years. The Barthel Index was used to assess the dependence on ADL and the Graffar Index on the socioeconomic level.Results: The data revealed a prevalence of 99.9% of MM in the elderly. Of these, 52.9% are female and the majority of them were in the age group 76-85 years (39.1%). Each elderly male has an average of 8.67 problems and the female 9.51 problems. The Barthel and Graffar Indexes´ filling are low, the Barthel Index is 7.7% and the Graffar Index is 5.2%. The elderly without MM have higher dependency rates than those with MM (the total dependence with 7.4% vs 6.2% and severe dependence 20.1% vs 9.6%). The elderly with MM are mainly at lower social classes, especially in the middle (55.3% vs 27.5%) and low (31.8% vs 19.6%), while the elderly without MM are mainly at higher social classes, especially at middle to upper (35.3%) and upper (17.6%).Conclusions: Higher MM is associated with lower social classes. Dependence on ADL appears to be unrelated to MM, according to the Barthel Index. The GP´s approach in the elderly´s consultation should be the MM. People with MM need special attention, based on their social and economic contexts, because their treatment must cover several chronic diseases simultaneously, with risks associated with polypharmacotherapy. The filling of the Barthel and Graffar Indexes seems to be low.
