Abstract

A dor crónica é condição prevalente em idosos e apresenta grande repercussão no estado de saúde desta população, sendo uma das queixas mais comuns nas instituições de cuidados de saúde.Com o aumento da esperança média de vida e das doenças crónicas, os cuidados paliativos passam a ter um papel vital na área de Medicina Geral, esta área da medicina consiste em cuidados de saúde activos que pretendem prevenir e minorar o sofrimento das doenças incuráveis num estado avançado e progressivo, integrando o controlo dos sintomas e apoio à família. A elevada prevalência de doenças sintomáticas nos indivíduos com mais de 65 anos leva a que esta população seja a que mais consuma um elevado número de fármacos, a variabilidade acentuada no processo de envelhecimento obriga a que o regime terapêutico seja individual de idoso para idoso, devendo eles receber tratamento apenas se absolutamente necessário, para indicações precisas e na menor dose eficaz. Este estudo tem como objectivo, identificar estratégias terapêuticas paliativas farmacológicas e não farmacológicas na idade geriátrica em doenças terminais neurodegenerativas, que possam ser aplicadas de forma a causarem menos efeitos secundários, efeitos estes que possam exacerbar a dor e inibir o seu alívio. Sendo um artigo de revisão, foi feita uma análise sistematizada em torno de 30 artigos científicos entre os anos 2000-2018, mas apenas 12 artigos contribuíram para o resultado deste resumo, as ferramentas utilizadas para a pesquisa destes artigos foram o sistema on-line de busca e análise de literatura médica MEDLINE, e sciELO.br efectuadas pesquisas na língua português assim como na língua inglesa, e Espanhola. As palavras-chave usadas nesta pesquisa foram, dor crónica, cuidados paliativos, doenças neurodegenerativas, geriatria, controlo da dor crónica. Conclui-se que embora a dor crónica seja um problema de saúde pública, são poucos os estudos feitos sobre o controle da dor na população geriátrica, uma vez que sendo a mais carenciada de cuidados por apresentar inúmeras comorbilidades derivadas a idade, dever-se-ia investir mais no estudo da dor nesta população, os estudos existentes também não apresentam uma eficácia 100% comprovada chegado alguns a serem inviáveis.Técnicas de controlo não farmacológico da dor cronica têm sido experimentados em muitas áreas da medicina, mas ainda pouco se sabe sobre a sua eficácia em idosos em estado terminal.

Chronic pain is a prevalent condition in the elderly and has great repercussions on the health status of this population, being one of the most common complaints in health care institutions. With the increase in the average life expectancy and chronic diseases, palliative care has a vital role in the area of General Medicine, this area of medicine consists of active health care intended to prevent and alleviate the suffering of incurable diseases in a state advanced and progressive, integrating symptom control and family support. The high prevalence of symptomatic diseases in individuals older than 65 years leads to this population being the one that consumes the highest number of drugs, the marked variability in the aging process requires that the therapeutic regimen be individual from the elderly to the elderly, and should they receive treatment only if absolutely necessary, for precise indications and at the lowest effective dose. The aim of this study is to identify pharmacological and non-pharmacological palliative therapeutic strategies in the geriatric age in neurodegenerative terminal diseases that can be applied in a way that causes less side effects, which may exacerbate the pain and inhibit its relief. As a review article, a systematized analysis was performed around 30 scientific articles between the years 2000-2018, but only 12 articles contributed to the result of this summary, the tools used for the research of these articles were the online system of search and analysis of medical literature MEDLINE, and sciELO.br carried out researches in the Portuguese language as well as in the English language, and Spanish. The keywords used in this research were: chronic pain, palliative care, neurodegenerative diseases, geriatrics, chronic pain control. It is concluded that although chronic pain is a public health problem, few studies have been done on pain control in the geriatric population, since it is the most deprived of care because it presents numerous comorbidities derived from age, would invest more in the study of pain in this population, the existing studies also do not present a 100% proven effectiveness some arrived to be infeasible. Non-pharmacological control techniques of chronic pain have been tried in many areas of medicine, but little is known about its efficacy in the terminally ill elderly.
