Abstract

Os linfomas são neoplasias resultantes da acumulação de linfócitos malignos nos gânglios linfáticos, podendo atingir o sangue (fase leucémica) ou infiltrar outros órgãos para além dos do tecido linfóide. Têm origem nas células do sistema linfóide em diferentes estádios de diferenciação. A maioria tem origem em células B e menos frequentemente em células T ou NK. Existem dois grandes subtipos principais: os linfomas Hodgkin (LH) e os linfomas não-Hodgkin (LNH). Os LNH são os tumores hematológicos mais comuns, representando actualmente a quinta neoplasia mais frequente no mundo. Contrariamente aos LH, a sua incidência tem vindo a aumentar em todos os grupos etários, excepto em crianças. Os conhecimentos sobre a patogénese molecular dos LNH têm aumentado significativamente nas últimas décadas tendo-se demonstrado que a imortalização celular se deve à acumulação de lesões genéticas e/ou epigenéticas, em conjunto com outros factores de risco tais como determinadas infecções, debilitação do sistema imunitário e diversos factores ambientais. A classificação mais actual dos linfomas é da Organização Mundial de Saúde (OMS), baseada na célula de origem e nas características histológicas, morfológicas, imunofenotípicas, genéticas/moleculares e clínicas. No entanto, não engloba a forma e o comportamento relativamente à evolução clínica. Com base nestas características os LNH são subdivididos em indolentes, agressivos e altamente agressivos. Os linfomas agressivos representam 65% do total de LNH e no geral apresentam uma evolução rápida, geralmente sintomática mas com melhor resposta à terapêutica do que os linfomas indolentes. Incluem diversas entidades histológicas, que se distribuem preferencialmente por diferentes faixas etárias e apresentam diferente comportamento clínico. Abrangem várias entidades histológicas distintas, que incluem o linfoma difuso de grandes células B, o linfoma de Burkitt, o linfoma linfoblástico, o linfoma de células do manto e o linfoma de grandes células anaplásico, entre outros. O avanço tecnológico das últimas décadas permitiu o estudo molecular de linfomas que apesar de histologicamente semelhantes apresentam comportamentos clínicos diferentes. Através desta análise foi possível construir modelos de prognóstico e novos alvos terapêuticos, tornando o tratamento dos LNH agressivos mais eficaz e personalizado. Este trabalho procura fazer uma revisão sobre o actual conhecimento da caracterização molecular/biológica e clínica dos LNH agressivos e qual a sua relevância clínico-terapêutica nestas neoplasias do sistema linfóide, dando especial atenção a alguns dos subtipos mais frequentes

Lymphomas result from cumulating malignant lymphocytes in the lymph nodes, that could reach the blood (leukemic phase) or infiltrate other organs beyond the lymphoid tissue. They derive from lymphoid system cells at different stages of differentiation. Most derive from B-cells and, less often, from T-cells or NK-cells. There are two major subtypes: Hodgkin (HL) and non-Hodgkin lymphomas (NHL). NHL are the most frequent hematological malignancies, currently being the fifth world's commonest cancer. In contrast with HL, its incidence has been increasing in all age groups, except in children. The knowledge about NHL's molecular pathogenesis have significantly increased in the last decades, concluding that cellular immortalization is due to genetic and/or epigenetic lesions, combined with other risk factors as infections, immunodeficiency and environmental factors. World’s Health Organization's (OMS) classification of lymphomas is the most current, based on cell origin and histological, morphological, immunophenotypic, genetic/molecular and clinical features. However it doesn't include clinical evolution. Based on these features, NHL are subdivided into indolent, aggressive and highly aggressive lymphomas. The aggressive lymphomas account for 65% of all NHL and in general progress rapidly, often symptomatic but respond better to therapy than indolent lymphomas. They comprise several histological entities (difuse large B-cell lymphoma, Burkitt's lymphoma, lymphoblastic lymphoma, mantle-cell lymphoma, anaplastic large cell lymphoma) and different clinical behavior, affecting specific age groups. Last decades' technological advances enabled the molecular analysis of lymphomas that exhibit different clinical behaviors although histologically similar. This allowed the definition of prognostic systems and new therapeutic targets, rendering the aggressive NHL treatment more efficient and individualized. This paper aims to review the current knowledge about aggressive NHL molecular/biological and clinical characterization and its clinical and therapeutical relevance, giving special attention to some of the most frequent subtypes
