Abstract

A ileíte terminal é uma patologia inflamatória da porção terminal do íleon, caracterizando-se por um espessamento da parede intestinal superior a 3mm, frequentemente detectado em ecografia abdominal. Embora alguns casos sejam assintomáticos, pode cursar com um quadro agudo ou crónico de dor abdominal no quadrante inferior direito e/ou diarreia, ou sintomas obstrutivos, hemorragia e manifestações extra-intestinais. No passado, era exclusivamente associada à doença de Crohn. No entanto, actualmente sabe-se que se encontra aliada a várias patologias. Assim sendo, o diagnóstico diferencial da ileíte terminal é um desafio à prática clínica do pediatra, sendo necessário uma história clínica e exame físico completos, bem como a realização de exames complementares de diagnóstico adequados e direccionados às suspeitas clínicas.Este projecto tem como objectivo realizar uma revisão da literatura dos diferentes achados e patologias associadas à ileíte terminal, tendo como foco de estudo o doente pediátrico. Para além disso, visará fornecer um melhor entendimento da mesma como entidade comum a diversas patologias, tanto do foro gastroenterológico como sistémico. Terminal ileitis is an inflammatory condition of the terminal portion of the ileum, characterized by thickening of the intestinal wall greater than 3 mm, often detected in abdominal ultrasound. Although some cases are asymptomatic, it can occur with as acute or chronic abdominal pain in the lower right quadrant and/or diarrhea, or chronic obstructive symptoms, hemorrhage and extra-intestinal manifestations.In the past, it was exclusively associated with Crohn's disease. However, it is now known that it is associated with several pathologies. Therefore, the differential diagnosis of terminal ileitis is a challenge to the pediatrician's clinical practice, requiring a complete clinical history and physical examination, as well as adequate complementary diagnostic tests directed at clinical suspicions.This project aims to carry out a literature review of the different findings and pathologies associated with terminal ileitis, focusing on the pediatric patient. In addition, it will aim to provide a better understanding of it as an entity common to several pathologies, both gastroenterological and systemic.
