Abstract

Introduction: The impact of psychosocial factors in Crohn’s Disease (CD) remains controversial. The aim of this study was to evaluate the role of depression and anxiety in the course of the disease. Methods: A cross-sectional and transversal analysis was performed including 80 patients - 40 men and 40 women - followed at the Gastroenterology outpatient clinic of Centro Hospitalar e Universitário de Coimbra, who were enrolled between September/2017 and December/2017. The symptoms of depression and anxiety were evaluated using the Hospital Anxiety and Depression Scale (HADS), the clinical activity of CD was defined according to the Harvey-Bradshaw Index (HBI), and the endoscopic activity of CD was evaluated through the Rutgeerts score and the Simple Endoscopic Score for Crohn’s Disease (SES-CD). Demographical and clinical data from the sample were also collected. Results: The median age of the patients was 37 years old (30-50). CD had a median duration of 13 years (9-18) and 88.8% of the patients were in remission according to HBI. Additionally, 86.3% of the patients were under immunosuppressive therapy. In total, 46.3% of the patients had mild to severe anxious symptoms, and 15.1% had mild to severe depressive symptoms. Both anxiety and depression revealed a statistically significant correlation with HBI score (p<0.05). The remaining clinical and analytical parameters did not present any evidence of association with HADS score. Discussion and Conclusion: Psychosocial factors such as anxiety and depression have an impact in the clinical activity of CD and may therefore influence the progression of the disease. Routine evaluation of depressive and anxious symptoms by physicians specialized in the treatment of CD patients may have a contributive role in CD management.

Introdução: O papel que os fatores psicossociais têm na Doença de Crohn (DC) é controverso. O objetivo deste estudo foi analisar o impacto da ansiedade e da depressão no curso da doença. Métodos: Realizámos um estudo de coorte, transversal, com 80 doentes - 40 homens e 40 mulheres - seguidos no Serviço de Gastrenterologia do Centro Hospitalar e Universitário de Coimbra, incluídos entre os meses de setembro e dezembro de 2017. Os sintomas de depressão e ansiedade foram avaliados pela Hospital Anxiety and Depression Scale (HADS), a atividade clínica da doença foi definida pelo Harvey-Bradshaw Index (HBI), e a atividade endoscópica foi definida pelo score de Rutgeerts e pelo Simple Endoscopic Score for Crohn’s Disease (SES-CD). Recolheram-se dados demográficos e clínicos da amostra. Resultados: A mediana de idades dos doentes foi de 37 anos (30-50), sendo que a doença tinha uma mediana de duração de 13 anos (9-18) e 88,8% dos doentes encontravam-se em remissão segundo o HBI. Adicionalmente, 86,3% dos indivíduos estavam medicados com imunossupressores. Do total da amostra, 46,3% apresentavam sintomas ansiosos ligeiros a graves, e 15,1% tinham sintomas depressivos ligeiros a graves. A ansiedade e a depressão apresentaram uma correlação estatisticamente significativa com o resultado do HBI (p<0,05). Os restantes parâmetros clínicos e analíticos não mostraram qualquer evidência de associação com o resultado na escala HADS. Discussão e Conclusão: Fatores psicossociais como a ansiedade e a depressão têm impacto na atividade clínica da DC podendo, por isso, influenciar a progressão da doença. A avaliação rotineira pelos médicos que tratam os doentes com DC dos sintomas de depressão e ansiedade, poderá apresentar um papel importante na abordagem da doença.
