Abstract

Sudden death due to digestive pathology accounts for about 10 to 20% of all causes of sudden death. In this context, considering the multiple causal situations, the most relevant in national clinical practice are upper digestive hemorrhage caused by rupture of esophageal varices, upper digestive hemorrhage due to peptic ulcer and peptic ulcer perforation.Therefore the aim of this study is to characterize them, considering pathophysiology, clinical features, diagnosis, risk factors and treatment/prevention; as well as to indicate other causes of sudden digestive death.Thus, a review of the available national and international literature on the topic of sudden death originated in the digestive system, was performed.As so, 53 articles on sudden death of the digestive cause were found, of which 34 articles were analyzed based on clinical cases and 9 scientific articles. The conclusions observed during this anlysis are explored and systematized acording important variables.Among the several predisposing factors we emphasize the excessive alcohol consumption, advanced age and use of nonsteroidal anti-inflammatory drugs and acetylsalicylic acid.After this review, we recognized the lack of large population-based studies, both nationally and internationally, on sudden digestive death, remaining still uncertain its causes, prevalence, incidence, associated comorbidities or habits.Keywords: Sudden digestive death; Digestive pathology; Esophageal variceal hemorrhage; Peptic ulcer hemorrhage; Peptic ulcer perforation.

Estima-se que a morte súbita decorrente de patologia digestiva corresponda a cerca de 10 a 20% de todas as causas de morte súbita. Neste contexto, as situações causais são inúmeras, destacando-se, pela sua preponderância na prática médico-cirúrgica nacional, a hemorragia digestiva alta por ruptura de varizes esofágicas, a hemorragia digestiva alta por úlcera péptica e a perfuração de úlcera péptica. O objectivo deste estudo é caracterizá-las, tendo em conta a sua fisiopatologia, clínica, diagnóstico, factores de risco e tratamento/prevenção; bem como indicar outras causas de morte súbita digestiva.Para tal, procedeu-se à revisão da literatura disponível, quer nacional quer internacional, sobre o tema de morte súbita com origem no sistema digestivo. Assim, foram encontrados 53 artigos sobre morte súbita de causa digestiva, dos quais foram analisados 34 artigos baseados em casos clínicos e 9 artigos científicos.De entre os vários factores predisponentes são de realçar o consumo alcoólico excessivo, idade avançada e utilização de anti-inflamatórios não esteróides e ácido acetil-salicílico.Findo este trabalho, reconheceu-se a escassez de estudos de grandes dimensões, tanto a nível nacional como internacional, sobre a morte súbita digestiva, permanecendo incertas as suas causas, prevalência, incidência, comorbilidades ou hábitos associados.Palavras chave: Morte súbita digestiva; Patologia digestiva; Hemorragia por varizes esofágicas; Hemorragia por úlcera péptica; Perfuração de úlcera péptica.
