Abstract

A artroplastia invertida do ombro é uma opção válida para o tratamento de cada vez mais patologias. O objectivo deste artigo é reunir a informação mais relevante já publicada sobre o tema e estimular a pesquisa neste campo de trabalho, para melhorar os resultados e diminuir as complicações pós-cirúrgicas nos doentes submetidos a esta intervenção. Aqui, são discutidas as principais indicações para a artroplastia invertida do ombro, a comparação entre a abordagem cirúrgica deltopeitoral e transdeltóide, assim como as vantagens e desvantagens de cada uma, as complicações pós-cirúrgicas mais comuns, os resultados funcionais e o plano recomendado de mobilização precoce do ombro. A artroplastia invertida do ombro mostrou, ao longo dos anos, melhorar vários aspectos da vida dos doentes, nomeadamente a amplitude da mobilização activa da articulação e a sensação de dor, sendo esta melhoria mais marcada nos doentes com patologia da coifa dos rotadores. As abordagens cirúrgicas mais utilizadas são duas e não foram provadas diferenças nos seus resultados finais, tornando a experiência do cirurgião e da sua equipa o factor determinante na escolha de uma ou outra. Quanto às complicações intra e pós-cirúrgicas, estas não apresentam um impacto negativo relevante na vida dos doentes, pelo menos a curto/médio prazo. Para se esclarecer a sua influência nos resultados a longo prazo, são necessários estudos com período de acompanhamento mais alargado. Assim, mais estudos e investigações deverão ser realizados, de modo a melhorar a técnica de implantação, optimizar o procedimento cirúrgico e torná-lo mais preciso.

Reverse shoulder arthroplasty is a valid option for the treatment of several pathologies. The objective of this article is to gather the most relevant information already published on the subject and to stimulate research in this field of work, to improve the outcomes and to reduce surgical complications in patients with this intervention.In this article, are discussed the main indications for reverse shoulder arthroplasty, the comparison between the deltopectoral and transdeltoid surgical approach, as well as the advantages and disadvantages of each, the most common postoperative complications, the functional outcomes and the recommended early shoulder mobilization.Over the years, reverse shoulder arthroplasty has improved many aspects of patients' lives, including the extent of active joint mobilization and pain sensation, and this improvement is most marked in patients with rotator cuff disease.There are two most commonly used surgical approaches. Among them, no differences were shown in the final outcomes, making the surgeon and his team’s experience the main factor in choosing one or the other.As for intra and post-surgical complications, these do not have a relevant negative impact on the patient's life, at least in the short / medium term. In order to clarify its influence on the long-term results, researches with a longer follow-up period are necessary.Thus, further researches and investigations should be performed in order to improve the implantation technique, optimize the surgical procedure and make it more precise.
