Abstract

A surdez neurossensorial (SN) resulta da falência em converter vibrações em impulsos nervosos na cóclea. A SN tem uma prevalência estimada de 1 em 1000 recém-nascidos, sendo que 1 em cada 300 é afectado de forma ligeira. A sua etiologia pode ser classificada em congénita ou adquirida e por sua vez, a surdez congénita pode ser dividida em genética (sindrómica ou não sindrómica) e não genética. Apesar dos avanços na biologia molecular terem permitido identificar cada vez um maior número de genes associados à surdez neurossensorial, as infeções perinatais continuam a representar a etiologia de uma grande parte dos casos em idade pediátrica.O impacto desta patologia no neurodesenvolvimento é evidente dada a importância dos estímulos auditivos na aprendizagem. Entre as principais perturbações do neurodesenvolvimento associadas à surdez neurossensorial encontram-se as do desenvolvimento intelectual, as da comunicação e as motoras. Estas podem limitar a aprendizagem e interferir no desempenho académico, mas também causar isolamento social. A identificação da etiologia e de perturbações do neurodesenvolvimento associadas à surdez neurossensorial afeta a estratégia terapêutica, e tem um valor prognóstico significativo. O seu conhecimento é imprescindivel num processo de aconselhamento genético. Este trabalho teve como objetivo uma revisão da literatura existente sobre SN em idade pediátrica, tendo-se analisado informação relativamente à etiologia, epidemiologia e o seu impacto no neurodesenvolvimento. Assim, efetuou-se uma pesquisa com base num conjunto de artigos científicos e artigos de revisão, maioritariamente publicados nos últimos 10 anos (a partir de 2006), tendo consultado motores de busca como o Pubmed ou a Medline.

Sensorineural hearing loss results from failure to convert vibrations into nerve impulses in the cochlea. Sensorineural hearing loss has an estimated prevalence of 1 in 1000 newborns, and 1 in 300 is mildly affected. Etiology can be classified as congenital or acquired, and congenital hearing loss can be divided into genetics (syndromic or non-syndromic) and non-genetic. Despite latest advances in molecular biology that allowed to identify an increasing number of genes associated to sensorineural hearing loss, perinatal infections still represent the main cause. Sensorineural hearing effect on neurodevelopment is obvious given the importance of auditory stimuli in learning. The most frequent are cognitive, communication and motor disorders witch can limit the learning process and interfere with academic performance and also cause social isolation and behavior problems. Etiology and neurodevelopmental disorders affect the therapeutic strategy and have a significant prognostic value. This knowledge is essential to genetic counseling.The aim of this study was to review the existing literature about sensorineural hearing loss in pediatric age, including etiology, epidemiology and impact on neurodevelopment. A research was carried out based on a set of scientific papers and review articles, mainly published in the last 10 years (from 2006), having consulted search engines such as Pubmed or Medline. .
