Tromboembolia Pulmonar: comorbilidade frequente no Cancro do Pulmão?

Abstract

The Pulmonary thromboembolism (PTE) is characterized by the occlusion of the pulmonary artery or one of its branches by a thrombus, being a frequent and serious complication of lung cancer and its treatments, especially in advanced stages of the disease. In these patients, this condition is underdiagnosed and undertreated, which often means a very dark prognosis. Cancer patients who develop PTE have a shorter life expectancy, a decrease in quality of life, and may require postponing or even discontinuing cancer treatment. Therefore, it is urgent to prevent this complication in patients with lung cancer. The institution of primary prophylaxis with anticoagulants in patients with a high risk of developing pulmonary thromboembolism has been proposed by several authors. However, the use of these drugs in outpatients, especially in those under chemotherapy treatment, is still not consensual. Accordingly, a risk stratification of PTE in lung cancer patients is necessary in order to evaluate the situations where the benefit of anticoagulant therapy exceeds the risk of haemorrhagic complications and to make sure this prophylactic measure is cost-effective. This review aims to gather information from several studies that have been carried out in order to understand which patients with lung cancer have a higher risk of developing PTE, the implications of this condition in the course of the disease, which patients would benefit from primary antithrombotic prophylaxis and whether this is already a viable option.
