Abstract

Portugal tem apresentado índices de tuberculose superiores à média Europeia. No entanto, desde 2014, é considerado um país de baixa incidência. Após sucessivos problemas no fornecimento da vacina BCG, não disponível em Portugal desde maio de 2015, foi repensada a estratégia vacinal, sendo apenas vacinadas as crianças pertencentes a grupos de risco devidamente assinalados.A tuberculose ganglionar é um diagnóstico difícil em Pediatria, pois há dificuldade na obtenção de amostras adequadas para bacteriologia, e muitas vezes as lesões são paucibacilares.Relata-se um caso clínico de uma lactente de 10 meses, sem critérios para realização da vacina BCG, que foi avaliada em contexto de adenite cervical que não respondeu a vários cursos de antibioticoterapia. Constituiu um desafio diagnóstico, pois o estudo exaustivo não foi conclusivo. Apenas o teste de sensibilidade à tuberculínica, reforçado pela anatomia patológica sugestiva, permitiu estabelecer o diagnóstico provável de tuberculose ganglionar, tendo sido iniciada terapêutica antibacilar com bons resultados. Este caso pretende ilustrar um diagnóstico atualmente raro e difícil, e ainda questionar as novas medidas adotadas relativamente à não vacinação universal das crianças com a vacina BCG em Portugal. .

Portugal has been one of the countries with the highest burden of tuberculosis Europe. However, since 2014, Portugal has been considered a low incidence country. After successive problems with the supply of the BCG vaccine, unavailable in Portugal since May 2015, the vaccination strategy was rethought and currently only children belonging to risk groups are vaccinated.Tuberculous lymphadenitis is a difficult diagnosis in Paediatrics, as it is difficult to obtain adequate samples for microbiology and, often, lesions are paucibacillary.We report the clinical case of a 10 month old infant, without criteria for BCG vaccine, who presented with cervical lymphadenitis that did not respond to several courses of antibiotic therapy. This was a diagnostic challenge, since its extensive study was inconclusive. Only the tuberculin test, reinforced by suggestive pathology results, allowed to establish the probable diagnosis of tuberculous lymphadenitis, and antituberculosis therapy was started, with good results.This case aims to illustrate a currently rare and difficult diagnosis, and to question the new measures that were adopted for BCG vaccine concerning the universal non-vaccination of children in Portugal. .
