Abstract

Introdução: O elevado número de casos de Hipertensão Arterial em consultas de Medicina Geral e Familiar e a elevada prevalência e incidência da Hipertensão Arterial a nível mundial, bem como o desenvolvimento de Disfunção Eréctil em pacientes hipertensos justifica a crescente necessidade de detecção precoce desta patologia, de modo a implementar medidas de prevenção e/ou terapêuticas que retardem a progressão da doenca. Objectivos: Revisão da literatura sobre os mecanismos, diagnóstico e tratamento da Disfunção Eréctil e esclarecimento da relação entre esta e a Hipertensão Arterial. Desenvolvimento: A Disfunção Eréctil é definida como uma incapacidade de iniciar ou manter uma erecção que permita uma relação sexual satisfatória, exercendo um sério impacto negativo na qualidade de vida dos pacientes e dos seus parceiros. Embora não seja o único factor a contribuir para o desenvolvimento de Disfunção Eréctil, a disfunção endotelial parece ser o elo de ligação entre a Hipertensão Arterial e a Disfunção Eréctil. O sistema vascular do pénis é afectado pela Hipertensão Arterial de forma semelhante à restante árvore vascular sistémica, tornando-se importante uma vigilância apertada e uma detecção e controlo precoces desta patologia. A Disfunção Eréctil é muito comum em pacientes hipertensos, muitos deles seguidos em consultas de Medicina Geral e Familiar, de tal forma que o médico deve estar alerta para esta condição e tomar a iniciativa de abordar o paciente quanto à sua função sexual. O diagnóstico pode apoiar-se na utilização de vários testes mas deverá ter como base uma história clínica e um exame físico exaustivos. O tratamento varia de paciente para paciente, tem que ser personalizado e adaptado a cada caso, nomeadamente à existência de comorbilidades Conclusão: É importante um controlo rigoroso dos pacientes hipertensos, bem como a identificação, prevenção e/ou tratamento de factores de risco que possam contribuir para o desencadeamento ou agravamento da Disfunção Eréctil. A isto deve associar-se um seguimento próximo destes doentes de modo a detectar alterações estruturais e funcionais numa fase mais precoce da doenca.

Introduction: There are big number of cases of Arterial Hypertension among the primary care centers and a great worldwide incidence and prevalence of this pathology. This, as well as the development of Erectile Dysfunction in hypertensive patients, justifies the need of an early detection of this condition, so we can implement preventive or therapeutic measures that can delay the disease progression. Objectives: This article aims to review the literature referring to the mechanisms, diagnosis and treatment of Erectile Dysfunction and the clarification of the relation between this condition and Arterial Hypertension. Conclusions: A tight control of hypertensive patients as well as the identification, prevention and/or treatment of risk factors that can lead to or aggravate Erectile Dysfunction is extremely important. This ought to be associated with a careful follow-up so we can detect functional and structural modifications in the early stages of disease. Development: Erectile Dysfunction is defined as the inability to achieve or maintain an erection that allows a satisfactory sexual intercourse. It seriously affects the quality of life of the patient and his partner. Endothelial Dysfunction seems to be the link between Arterial Hypertension and Erectile Dysfunction, although it is not the only one. Arterial Hypertension affects penile arteries in the same way as systemic arteries. Thus, it is important to initiate an early surveillance, detection and control of this disease. Erectile Dysfunction is very common among hypertensive patients. Many of them are followed in primary care consultations. So the physician must be conscious of this condition and should be prepared to ask his patient about his sexual function. The diagnosis can be supported by many tests but the clinical history and physical examination of the patient should constitute the basis of it. The treatment is variable from one patient to another. It must be personalized and adapted to each case, paying special attention to the presence of comorbidities
