Abstract

Introdução: A afecção isolada da articulação patelo-femoral nos processos degenerativos do joelho é rara, encontrada em aproximadamente 3,8% dos joelhos artrósicos. O tratamento ideal para a condromalácia e/ou artrose patelo-femoral permanece desconhecido. Em 1963, Maquet propôs para a condromalácia e/ou artrose patelo-femoral dolorosa, resistente ao tratamento conservador, uma filosofia de tratamento cirúrgico diferente, realizando o avanço isolado da tuberosidade anterior da tíbia (TAT) em cerca de 2-2,5cm. A lógica deste procedimento era a redução do vector da força de compressão patelo-femoral, pelo aumento da relação angular entre o tendão patelar o tendão do m. quadriceps femoris. Objectivo: A realização deste trabalho pretendeu avaliar os resultados a longo prazo, tanto sob a perspectiva funcional como sob a perspectiva da ausência de sintomatologia, de doentes com condromalácia e/ou artrose patelo-femoral, submetidos à osteotomia de Maquet modificada (1,5-2.0cm). Metodologia: Numa amostra final de 17 indivíduos (17 joelhos) de ambos os géneros, com uma média de idades, aquando da operação, de 40 anos, realizamos uma consulta de revisão com um follow-up mínimo de 20 anos e máximo de 28 anos, onde foi feita uma avaliação clínica através da recolha de dados, objectivos e subjectivos, considerados mais relevantes e uma avaliação radiográfica de cada doente. Resultados: A taxa de resultados bons a excelentes foi igual a 60% segundo a avaliação COX e o alívio sintomático da dor ocorreu em todos os casos em que existia dor prévia à cirurgia, sendo que o seu reaparecimento só ocorreu em 53% dos casos ao fim de quase 20 anos. Doentes com menor incapacidade segundo a escala de Kujala, tiveram uma tradução radiográfica de gonartrose menos marcada. Conclusão: A osteotomia de Maquet modificada com elevação da TAT de 1,5-2,0cm constitui-se ainda como uma alternativa a ter em conta para o tratamento dos casos de condromalácia e/ou artrose patelo-femoral em adultos jovens, resistentes ao tratamento conservador, por obter bons resultados no alívio sintomático da dor anterior do joelho.

Background: The isolated affection of the patellofemoral joint in the knee degenerative processes is rare, found in approximately 3.8% of the arthritic knees. The ideal treatment for chondromalacia and/or patellofemoral osteoarthritis remains unknown. In 1963, Maquet proposed for painful chondromalacia and/or patellofemoral osteoarthritis, resistant to the conservative treatment, a different philosophy of surgical treatment, performing a isolated advancement of the tibial tuberosity in about 2,0-2,5cm. The rationale of this procedure rests on the reduction of the patellofemoral compressive force vector, by the increase in the angular relationship between the patellar tendon and the tendon of quadriceps muscle. Objectives: This work aimed to evaluate the long-term results, both in the functional perspective as the perspective of absence of symptoms, in patients with chondromalacia and/or patellofemoral osteoarthritis underwent modified Maquet Osteotomy (1,5-2,0cm). Methods: In a final sample of 17 individuals (17 knees) of both sexes, with a mean age of forty years old at the time of the surgery, we performed a review consultation with a followup of at least 20 years and maximum 28 years, where an assessment was made by collecting clinical data, subjective and objective, considered more relevant and radiographic evaluation of each patient was made to. Results: The rate of good to excellent results was of 60% according to the COX evaluation and the pain relief occurred in all cases with previous pain to the surgery, and its reappearance occurred in only 53% of cases after almost 20 years. Patients with less disability according to the Kujala scale had a less marked translation of radiographic gonarthrosis Conclusions: The modified Maquet Osteotomy, with an advancement of the anterior tibial tuberosity in about 1,5-2,0cm, still remains an alternative to the treatment of young adults with chondromalacia and/or patellofemoral osteoarthritis, resistant to the conservative treatment, because this procedure obtained good results in symptomatic relief of anterior knee pain
