Abstract

As neoplasias cerebrais condicionam uma elevada taxa de mortalidade e mau prognóstico. A terapia médica das neoplasias cerebrais constitui um enorme desafio à Medicina moderna uma vez que a maioria dos agentes terapêuticos não atinge o parênquima cerebral porque não consegue atravessar a barreira hemato-encefálica. Esta barreira apresenta se como uma entidade paradoxal, sendo fisiologicamente protectora e, simultaneamente, um obstáculo terapêutico. Consequentemente, a disrupção transitória, reversível e localizada desta barreira, associada à administração de diversos agentes farmacológicos, constitui a base conceptual para diversas estratégias terapêuticas. Através da análise dos resultados de diversos estudos e artigos de revisão acerca das várias técnicas disruptivas, pretende o autor sumariar as principais características de diversas técnicas disruptivas; verificar se existe disrupção da BHE e se existe aumento da concentração parenquimatosa aquando da associação de agentes terapêuticos; contribuir para um aumento do conhecimento relativamente à existência da necessidade e do papel que estas técnicas assumem no tratamento de tumores cerebrais. A análise de diversos estudos relativos a diferentes técnicas disruptivas demonstra a pertinência da disrupção da barreira hemato-encefálica enquanto entidade terapêutica, demonstrando um controlo na progressão tumoral e aumento da sobrevida sem progressão da doença. No entanto, é também premente a necessidade de mais estudos clínicos que avaliem a eficácia e, acima de tudo, a segurança dos diversos métodos disruptivos. Além disso, a investigação futura no que concerne à terapêutica de neoplasias cerebrais deverá abordar de forma holística não só a problemática imposta pela BHE, mas também a capacidade de captação de fármaco pelo tumor, o metabolismo intra-celular do fármaco, o washout do agente para o meio extra-celular ou para o LCR, a própria sensibilidade à quimioterapia e a existência de mecanismos de resistência. A compreensão da proteómica e genómica da BHE também assume um papel fundamental na investigação futura permitindo um consequente melhor entendimento acerca da cinética neuro-farmacológica.

Brain cancer is accompanied by high mortality rate and dismal prognosis. Its medical treatment presents as a major challenge to modern medicine as most therapeutic agents cannot reach the brain tissue due to their inability to cross the blood-brain barrier. The BBB presents as a paradoxal structure, physiologically protective and, simultaneously, a therapeutic hindrance. Thus, the transient, reversible and focused disruption of this barrier, combined with the administration of therapeutic agents, builds the conceptual basis for various treatment strategies. Through the analysis of the results of published studies and review articles concerning the different disruptive techniques, the author intends to summarize the main characteristics of some of these techniques and partake in the increase of the knowledge about the role that these play as therapeutic strategies for brain cancer. The analysis of several studies shows the importance of these BBB disruptive techniques as potential therapeutic maneuvers, showing control in tumor progression and increasing the progression-free survival. Nevertheless, more clinical studies are needed for evaluation of the efficacy and, above all, the security of these disruptive techniques
