Abstract

A Vitamina D tem sido estudada devido à sua influência sobre variadas doenças, entre as quais a Doença Alérgica. Tem-se verificado uma prevalência crescente no défice de Vitamina D, resultante de uma alimentação pobre nesta vitamina associada a uma diminuída exposição solar. Concomitantemente, tem-se verificado também um aumento na prevalência da Doença Alérgica. Este trabalho afigura-se oportuno, enquanto revisão de literatura, por avaliar a relação da Vitamina D com a Doença Alérgica, nomeadamente, a nível dos seus factores de risco, da sua patogénese, fisiopatologia, expressão clínica e terapêutica. A vitamina D é apontada por induzir a síntese de peptídeos antibacterianos (como a Catelicidina e a Defensina β-4), por alterar o equilíbrio Th1/Th2, por estimular a síntese de células T reguladoras, por interferir na maturação e na diferenciação de Células Dendríticas por alterar o padrão de síntese de citocinas, entre muitas outras coisas. A Vitamina D tem, consequentemente, um papel importante na determinação das características fenotípicas da asma, tais como a função pulmonar, a hiperreactividade das vias aéreas, o risco de exacerbação e a resposta à terapêutica. A elaboração de mais investigação, que permita chegar a conclusões com elevado grau de evidência, irá possibilitar o estabelecimento de um papel mais relevante para a Vitamina D e para os seus análogos na prática clínica da Doença Alérgica

Vitamin D has been studied due to its influence on various diseases. There has been an increase in vitamin D deficiency, resulting from a poor dietary intake associated with a reduced exposure to sunlight. Concomitantly, there has been an increase in the prevalence of allergic diseases. The objective of this paper is to evaluate the association between vitamin D and allergic diseases, focusing on the allergic diseases risk factors, their pathogenesis and pathophysiology as well as their clinical manifestations and therapy. Vitamin D is known to induce the synthesis of antibacterial peptides (such as defensin-β4 and cathelicidin), to modulate the Th1/Th2 balance, to stimulate the synthesis of regulatory T cells, to interfere with the maturation and differentiation of dendritic cells and to modulate the pattern of cytokine response, among other inflammatory phenomena. Vitamin D has an important role in determining the phenotypic characteristics of allergy, such as lung function, airway hyperresponsiveness, risk of exacerbation and therapeutic response. The development of further research, allowing the reaching of conclusions with a high degree of evidence, will enable the establishment of a more meaningful role for Vitamin D and its analogues in the clinical practice when dealing with allergic diseases.
