Abstract

Em 1978, a Declaração de Alma-Ata definiu os cuidados de saúde primários como a chave para o desenvolvimento económico e social, melhor qualidade de vida e paz mundial. Para atingir estes objetivos, a declaração defende princípios como a acessibilidade, uso adequado de recursos, equidade e justiça social. O modo como os princípios revolucionários de Alma-Ata foram aplicados não permitiu que se atingissem os objetivos de “Saúde para Todos no Ano 2000”, e assim, surgem no ano de 2000 os “Objetivos de Desenvolvimento do Milénio” com metas concretas a serem atingidas pelos países membros das Nações Unidas até ao ano de 2015. Os cuidados de saúde primários devem desempenhar um papel central nos sistemas de saúde, o que nem sempre aconteceu, havendo tendencialmente maior atenção dada aos serviços especializados e uma coexistência de múltiplos programas dirigidos, o que levou muitas vezes a menor acessibilidade e qualidade dos cuidados de saúde. É importante colocar os Cuidados de Saúde Primários no centro dos sistemas de saúde, de modo a que possa haver uma coordenação de serviço e que se possam prestar cuidados de saúde de qualidade e custo-efetivos com bons resultados nos indicadores de saúde. A Medicina Geral e Familiar é a especialidade médica que presta estes cuidados de saúde abrangentes, que levam em conta o contexto familiar e comunitário do doente, e cuja definição e características vão evoluindo de modo a que se possa alcançar cada vez mais cuidados de saúde de qualidade. A formação de médicos especialistas em Medicina Geral e Familiar é feita de acordo com o programa de formação pós-graduada de cada país, sendo de crucial importância que a especialidade tenha também forte presença na fase de formação pré-graduada para que se possam formar médicos capazes de prestar cuidados de saúde de qualidade.

In 1978 the Declaration of Alma-Ata defined primary health care as the key to economic and social development, a better quality of life and to world peace. To achieve these goals, the statement advocates principles such as accessibility, appropriate use of resources, equity and social justice. The way the revolutionary principles of Alma-Ata were used did not allow reaching the goals of "Health for All by the Year 2000", and thus in 2000 the "Millennium Development Goals" appear with specific objectives to be achieved by member countries of the United Nations by the year 2015. Primary health care should play a central role in health systems, which didn’t always happened. There tends to be greater attention given to specialized services and a coexistence of multiple targeted programs, which often led to lower accessibility and quality of health care. It is important to place the primary health care in the center of health systems, so that there may be a coordination of services and which may provide quality health care and cost-effectiveness with good results in health indicators. General Practice/ Family Medicine is the medical specialty that provides this comprehensive health care, taking into account the family and community context of the patient, and whose definition and characteristics will evolve so that we can reach better health care quality. The training of specialists in General Practice/Family Medicine is made in accordance with the program of postgraduate training of each country, being of crucial importance that the specialty has also a strong presence on the stage of pre-graduate training so that they can form physicians capable of providing quality health care.
