Abstract

Introdução - A existência de reactividade cruzada entre aeroalérgenos e alérgenos alimentares é conhecida. A síndrome ácaros-marisco caracteriza-se pelo aparecimento de alergia a marisco em doentes sensibilizados a ácaros e a síndrome pólenes - frutos por alergia a frutos e vegetais frescos em doentes sensibilizados a pólenes. As manifestações destas alergias podem ir de sintomas localizados e ligeiros, como na síndrome de alergia oral, até casos graves de anafilaxia. O risco de desenvolver alergia a alimentos com reactividade cruzada em doentes sensibilizados a alérgenos ambientais é uma questão pertinente para pais e profissionais de saúde, pela necessidade de aconselhar medidas de evicção alimentar. Objectivo – Identificar padrões clínicos e/ou laboratoriais sugestivos de desenvolvimento de alergia a frutos e vegetais frescos e alergia a mariscos, em doentes sensibilizados a pólenes e a ácaros, respectivamente. Métodos - Em doentes com prováveis síndromes de reactividade cruzada alimentos/ aeroalérgenos, foram analisados dados da história pessoal, os valores de IgE específicas a frutos/vegetais, ácaros e pólenes e IgEs específicas para componentes alergénicos através da técnica de microarray. Artigo Científico Resultados – Metade dos doentes apresentavam asma, rinite e dermatite atópica aquando do aparecimento da alergia alimentar. Metade tinha história de outras alergias ou sensibilizações a outros alimentos. Os valores de IgEs específicas a pólenes e ácaros eram baixas. Apesar de reacções graves, as IgE específicas para os frutos e mariscos eram baixas, especialmente no caso das frutas. A determinação de IgEs específicas para componentes alergénicos comprovou reactividade cruzada em 7/14 doentes podendo antecipar nestes a possibilidade de desenvolverem alergia a frutos/ vegetais frescos ou marisco. Conclusão– Valores baixos de IgEs específicas a ácaros, pólenes, marisco e frutos/vegetais não afasta o risco de desenvolver alergia alimentar. Embora com algumas limitações, a utilização do diagnóstico molecular através da técnica de microarray permite identificar doentes com risco de alergia alimentar.

Introduction: The existence of cross-reactivity between aeroallergens and food allergens is known. The seafood-mite syndrome is characterized by appearance of seafood allergy in patients sensitized to dust mites and the fruits-pollen syndrome by allergy to fresh fruits and vegetables in patients sensitized to pollens. The manifestations of these allergies can range from mild and localized symptoms, such as oral allergy syndrome, to severe cases of anaphylaxis. The risk of developing food allergy with cross-reactivity in patients sensitized to environmental allergens is a relevant issue for parents and health professionals, who need to advise food avoidance measures. Objectives: Investigate clinical patterns and/or laboratory findings, suggestive of developing allergies to fruits and vegetables and seafood allergy, in patients sensitized to pollens and mites, respectively. Methods: In patients with probable food/aeroallergens cross reactivity syndromes, we have analyzed data from personal history, specific IgE values of fruit/vegetables, mites and pollens and specific IgE antibodies against allergenic components were measured via microarraAry technique. Results: Half of the patients had concomitant asthma, rhinitis and atopic dermatitis on occurrence of food allergy. Half had history of other allergies or sensitization to food and/or aeroallergens. The specific IgEs values to mites and pollens were low. Specific IgEs to food were low, even with severe reactions, especially in the case of fruits. The determination of specific IgE antibodies against allergenic components demonstrated cross reactivity in 7/14 patients, meaning that in those cases we can anticipate a possible occurrence of allergy to fresh fruits and vegetables or to seafood. Conclusion: The low values of specific IgEs in the serum to aero and food allergens does not eliminate the risk of developing food allergy. Although with some limitations, the use of microarray technique can identify patients at higher risk of food allergy
