Abstract

A fenda lábio-palatina constitui a mais comum malformação congénita da cabeça e do pescoço e apresenta uma prevalência global estimada de 1/700 nados-vivos. Esta malformação assume uma grande diversidade clínica e morfológica e resulta de anomalias embriológicas de etiologia multifactorial, que decorrem durante as primeiras semanas de gestação. O tratamento das fendas lábio-palatinas deve ser iniciado logo após o nascimento, prolongase até à idade adulta e exige a actuação de uma equipa multidisciplinar. O tratamento inadequado desta malformação acarreta elevadas consequências para o indivíduo. Existem diversas opções terapêuticas disponíveis, mas a sua escolha varia de acordo com o local de tratamento, experiência e preferência pessoal do médico/cirurgião e características do indivíduo. O presente artigo apresenta uma revisão científica das características gerais das fendas lábiopalatinas e do seu tratamento, com ênfase na vertente cirúrgica, realizada com base na literatura mais recente, descrevendo, analisando e comparando diferentes técnicas como a reparação primária do lábio e palato, o enxerto ósseo alveolar, a cirurgia ortognática e a distracção óssea, entre outras.

Cleft lip and palate is the most common head and neck congenital malformation and has an estimated global prevalence of 1 per 700 live births. This malformation assumes a great clinical and morphological diversity and is caused by embryological abnormalities of multifactorial aetiology which occur during the first weeks of gestation. Cleft lip and palate treatment should be initiated right after birth, continues until adult age and demands a multidisciplinary team interaction. An inadequate treatment of this malformation entails great consequences for the individual. There are several treatment options available but its selection varies according to the place of treatment, doctor’s or surgeon’s personal experience and preference and subject’s characteristics. This article presents a scientifical review of cleft lip and palate’s general characteristics and treatment, based on the most recent literature, emphasizing its surgical aspects, describing, analyzing and comparing different techniques such as primary lip and palate repair, alveolar bone graft, orthognathic surgery and distraction osteogenesis
