Abstract

The aim of this work consists on the numerical analysis of the clinical procedure that is used in the knee arthrodesis. This work studies the influence of the parameters that condition the process of compression of the shear surfaces in the femur and tibia bones, namely the type of support, assembly sequence, load intensity, positioning of the fixator and orthostatic positioning. The analysis of these parameters went through the development of CAD models in Solidworks® and numerical simulation of the respective models, using the software ADINA®. This methodology allowed the recreation of the procedure performed for the knee arthrodesis. The comparison of the results allowed the evaluation of the conditions that enable the success of the surgery, namely the fixation of the shear surfaces, the distribution of the Von Mises stresses in the femur and tibia bones, the homogeneity of the stresses of the contact surfaces and the resultant load on the compression focus. The results presented in this work indicate that, to obtain a better homogeneity in the shear area and lower slip between the limbs, the methodology that should be adopted involves the simultaneous grip of the clamps, the correct positioning of the fixator and a careful attention of the valgus angle for the positioning of the limbs.

O objetivo deste trabalho consistiu na análise numérica do procedimento clínico utilizado na artrodese do joelho. Neste estudo, foi avaliada a influência dos parâmetros que condicionam o processo de compressão das superfícies de corte nos ossos do fémur e da tíbia, tais como, o tipo de apoio, a sequência de montagem, a intensidade de carga, o posicionamento do fixador e o posicionamento ortostático. A análise dos parâmetros passou pelo desenvolvimento de modelos em CAD, utilizando o Solidworks®, e pela simulação numérica dos respetivos modelos, recorrendo ao programa ADINA®. Esta metodologia permitiu recriar o procedimento efetuado para a artrodese do joelho. A comparação dos resultados permitiu avaliar as condições que favorecem o sucesso da cirurgia, tais como, a fixação nas superfícies de corte, a distribuição das tensões de von Mises nos ossos do fémur e da tíbia, a homogeneidade das tensões na superfície de contacto e a força resultante no foco da compressão. Os resultados apresentados indicam que, para obter uma melhor homogeneidade na zona de corte e menor escorregamento entre os membros, a metodologia a adotar passa pela realização do aperto simultâneo dos clamps, pelo correto posicionamento do fixador e a cuidada atenção ao ângulo de valgos para o posicionamento dos membros.
