Abstract

Introdução: a diminuição da ingestão oral no doente em fim de vida é frequente. As evidências sobre alimentação e hidratação artificiais são divergentes, nomeadamente no seu contributo para a qualidade de vida dos doentes. Objetivos: analisar a preparação, atitudes e orientações dos médicos que trabalham nas organizações de cuidados paliativos em Portugal, relativamente à nutrição e hidratação em fim de vida. Avaliar o conhecimento e preparação dos alunos, em fase final da formação médica, para lidar com questões do doente em fim de vida. Métodos: foram elaborados e disponibilizados online dois questionários, através da ferramenta “Formulários do Google”. O “questionário 1” foi dirigido aos médicos que exercem em organizações de cuidados paliativos e o “questionário 2” dirigido aos alunos do 6.º ano médico. Resultados: um total de 20 médicos respondeu ao questionário. Destes, 95% refere ter formação em cuidados paliativos e 80% em nutrição. 70% consideram-se seguros para fazer um bom aconselhamento nutricional ao doente. Quanto aos alunos, foram obtidas 69 respostas de alunos do 6.º ano médico. 69,6% tiveram formação em cuidados paliativos e 81,2% em nutrição. 95,7% não se sentem preparados para lidar com questões dos doentes terminais. Conclusões: há uma boa cobertura formativa dos médicos associados aos cuidados paliativos, que se traduz numa boa resposta terapêutica. Os padrões de prescrição e suspensão de nutrição e hidratação medicamente assistidas são maioritariamente adaptados às necessidades do doente. A preparação dos alunos de medicina parece não estar a corresponder às exigências de uma população que continuará a envelhecer nos próximos anos.

Introduction: the reduction of oral intake of a patient at the end of his life is common. Evidence about artificial nutrition and hydration are deviating, especially in their contribution to increase life quality of patients. Objectives: analyze the preparation, attitudes and orientation of doctors working in palliative care in Portugal, regarding nutrition and hydration of a patient at the end of his life and to evaluate knowledge and preparation of students at the end of their medical career for treating patients at the end of their lives. Methods: Two questionnaires were elaborated and made available online by using Google Forms. The “questionnaire 1” was aimed at doctors who practice in palliative care and “questionnaire 2” at medical student in their 6th year. Results: A total of 20 doctors responded to the questionnaire. Among the respondents, 95% indicate to have further education in palliative care and 80% in nutrition. 70% consider themselves as confident in giving patients good nutritional advices. Regarding medical students 69 questionnaires were handed in by students of the 6th year. 69.6% had received an education in palliative care and 81.2% in nutrition. 95.7% do not feel prepared to handle the care of patients in final stages. Conclusion: There is a good covering concerning the formation of doctors associated in palliative care, leading to good therapeutic measures. The standards of prescription and suspension of medically-assisted nutrition and hydration are mostly adjusted to the patients needs. The preparation of the medical students doesn’t correspond with the demand of an increasingly aging population.
